家庭医生式服务模式对社区乳腺癌患者自我感受负担、负性情绪和生活质量的影响

The influence of family doctor service mode on self perceived burden,negative emotion and quality of life of breast cancer patients in community

目的:探究家庭医生式服务模式对社区乳腺癌患者自我感受负担、负性情绪和生活质量的影响。方法:2018年3月-2019年2月收治社区乳腺癌患者50例,随机数字表法分为两组各25例。两组均给予常规治疗方式,对照组给予常规社区干预,研究组给予家庭医生式服务。比较两组患者不良情绪评分(焦虑与抑郁情绪)、生活质量、自我感受负担变化情况。结果:两组治疗前自我负担感受、不良情绪评分比较,差异无统计学意义(P>0.05)。两组干预后自我负担感受、不良情绪评分均低于治疗前,且研究组显著低于对照组,差异有统计学意义(P<0.05)。两组治疗前躯体功能、角色功能、情绪功能、社会功能及认知功能评分比较,差异无统计学意义(P>0.05);两组干预后躯体功能、角色功能、情绪功能、社会功能及认知功能评分均低于治疗前,且研究组各项功能评分较对照组升高更明显,差异有统计学意义(P<0.05)。结论:推广以家庭医生服务为核心的社区卫生服务模式,对乳腺癌患者实施有效干预措施,对于降低患者自我感受负担、不良情绪及提高生活质量有重要意义,值得推广。

Objective:To explore the influence of family doctor service mode on self perceived burden,negative emotion and quality of life of breast cancer patients in community.Methods:From March 2018 to February 2019,50 breast cancer patients in the community were selected,and they were divided into two groups with 25 cases in each group with random number table method.Both groups received routine treatment,the control group was given routine community intervention,the study group was given family doctor service.We compared the changes of anxiety and depression,quality of life,and self perceived burden between the two groups.Results:There was no significant difference in the scores of self burden and bad emotion before treatment between the two groups(P>0.05).After intervention,the scores of the two groups were lower than that before treatment,and the study group was significantly lower than the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in scores of physical function,role function,emotional function,social function and cognitive function between the two groups(P>0.05).After intervention,all the functions of the two groups were lower than before treatment,and the rise of score of each function in the study group was more pronounced,the difference was statistically significant(P<0.05).Conclusion:It was of great significance for breast cancer patients to promote the community health service model with family doctor service as the core,it can reduce patients'burden of self feeling,negative emotions and improve their quality of life.
