掌骨牵引联合切开复位内固定术治疗重度尺桡骨下段陈旧性骨折的临床效果

Clinical effect of metacarpal traction combined with open reduction and internal fixation in the treatment of severe old fracture of lower ulna and radius

目的探讨掌骨牵引联合切开复位内固定术治疗重度尺桡骨下段陈旧性骨折的临床效果。方法选取2016年4月~2018年4月兴国县人民医院收治的52例重度尺桡骨下段陈旧性骨折患者作为研究对象,采用随机数字表法将其分为对照组和观察组,每组各26例。对照组患者给予常规治疗,观察组患者则给予掌骨牵引联合切开复位内固定术治疗。比较两组患者的治疗总有效率、神经功能缺损评分、疼痛评分及生活质量评分。结果观察组患者的治疗总有效率(96.15%)高于对照组(69.23%),神经功能缺损评分[(20.34±1.52)分]低于对照组[(46.61±2.06)分],疼痛评分[(2.06±0.51)分]低于对照组[(4.36±1.40)分],生活质量评分[(37.45±1.42)分]高于对照组[(28.61±2.51)分],差异均有统计学意义(P<0.05)。结论较常规治疗方法,掌骨牵引联合切开复位内固定术治疗重度尺桡骨下段陈旧性骨折患者的临床效果较好,可减少神经功能缺损,减少疼痛感,提高生活质量,值得临床应用及推广。

Objective To investigate the clinical effect of metacarpal traction combined with open reduction and internal fixation in the treatment of severe old fracture of lower ulna and radius.Methods From April 2016 to April 2018,52 patients with severe old fracture of lower ulna and radius treated in Xingguo People′s Hospital were selected as the research objects,and they were divided into control group and observation group with 26 cases in each group by random number table method.Patients in the control group were given routine treatment,while patients in the observation group were given metacarpal traction combined with open reduction and internal fixation.The total effective rate,neurological deficit score,pain score and quality of life score of the two groups were compared.Results The total effective rate(96.15%)of the patients in the observation group was higher than that in the control group(69.23%),the neurological deficit score([20.34±1.52]points)was lower than that in the control group([46.61±2.06]points),the pain score([2.06±0.51]points)was lower than that in the control group([4.36±1.40]points),and the quality of life score([37.45±1.42]points)was higher than that in the control group([28.61±2.51]points),the differences were statistically significant(P<0.05).Conclusion Compared with conventional treatment methods,metacarpal traction combined with open reduction and internal fixation has a better clinical effect in the treatment of patients with severe old fractures of lower ulna and radius,which can reduce neurological deficits,reduce pain and improve quality of life,and is worthy of clinical application and promotion.
