我院门诊处方审核中发现的问题及对策分析

Analysis on the problems and countermeasures found in the outpatient prescription audit in our hospital

目的分析我院门诊处方审核中常见的处方问题并探讨相应的解决方法。方法收集和整理2018年第四季度不合理的门诊处方,分析药师对其干预的情况。结果发现问题主要包括医师疏忽及手误导致的、患者原因导致的有潜在问题的、未备案的超说明书用药及因为技术问题出现的问题处方。药师对其中309张处方进行了干预,干预成功率为72.49%。结论对处方审核时发现的问题处方进行分析,能发现门诊问题处方的常见类型、出现频率及医师对此类问题反馈的接受程度,为规范门诊合理用药及进一步推行处方前置审核提供很好的参考。

Objective To analyze the common problems of outpatient prescription audit in our hospital and probe into the corresponding solutions.Methods The unreasonable prescriptions in the fourth quarter of 2018 were collected and classified,and the results of intervention by pharmacists were analyzed.Results The problems of unreasonable prescriptions included negligence of the doctors,matters caused by the patients,off-label drug use and technical factors.In addition,309 prescriptions amomg them were intervened by pharmacists and the intervention success rate was 72.49%.Conclusion Problems of the unreasonable outpatient prescriptions,incidence of each problem and the acceptance of the doctors to the intervention by pharmacists can be found by outpatient prescription audit,which can provide valuable reference for the rational use of drug for outpatients and implementing prescription audit.
