老年慢性心力衰竭患者生存状况的影响因素分析

Analysis on the influence factors of living condition in elderly patients with chronic heart failure

目的探讨老年慢性心力衰竭患者生存状况的影响因素,为改善其生活质量提供干预依据。方法选取2018年10月~2019年1月在济宁医学院附属医院兖州院区心内科门诊就诊的500例老年慢性心力衰竭患者作为研究对象,采用成年人健康自我管理能力测评量表(AHSMSRS)和自测健康评定量表(SRHMS)对其生存状况进行调查,采用分层回归分析法分析生存状况的影响因素。结果老年慢性心力衰竭患者AHSMSRS总得分为(128.26±11.42)分,SRHMS总得分为(341.71±18.33)分。不同年龄、婚姻、文化程度、职业、经济状况及健康自我管理能力情况下患者的SRHMS得分比较,差异有统计学意义(P<0.01)。回归分析结果显示,年龄、职业、婚姻、文化程度、经济状况、健康自我管理能力因素可共同解释生存状况62.3%的变异量(R2=0.623),其中,健康自我管理能力可单独解释患者生存状况32.5%的变异量(R2=0.325)。结论年龄、职业、婚姻、受教育程度、经济状况以及健康自我管理能力是影响老年慢性心力衰竭患者生存状况的因素,其中,健康自我管理能力的影响作用最大。

Objective To explore the influence factors of living condition in elderly patients with chronic heart failure,in order to provide the intervention basis for improving life quality.Methods Five hundred elderly patients with chronic heart failure who were admitted to the Department of Cardiology of Yanzhou Branch of Affiliated Hospital of Jining Medical University from October 2018 to January 2019 were selected as study subjects.The adult health self-management skill rating scale(AHSMSRS)and the self-rating health measurement scale(SRHMS)were used to investigate the living condition,and the hierarchical regression analysis was used to analyze the influence factors of living condition.Results The total score of AHSMSRS was(128.26±11.42)points and the total score of SRHMS was(341.71±18.33)points in elderly patients with chronic heart failure.There were statistically significant differences in the SRHMS scores of the patients under different age,marriage,education level,occupation,economic status and health self-management skill(P<0.01).Regression analysis showed that age,occupation,marriage,education level,economic status,and health self-management skill factors could explain the 62.3%variation in living condition(R2=0.623).Among them,health self-management skill could explain the 32.5%variation of patients′living condition separately(R2=0.325).Conclusion Age,occupation,marriage,education level,economic status and health self-management skill are factors influencing the living condition of elderly patients with chronic heart failure.Among them,the influence effect of health self-management skill is the greatest.
