风湿性疾病患者25-羟-维生素D水平与心血管疾病的关系分析

Analysis of Relationship between 25-hydroxy-vitamin D Level and Cardiovascular Disease in Patients with Rheumatic Diseases

摘要 目的研究风湿性疾病患者25-羟-维生素D水平与心血管疾病的关系。方法该院选取2017年5月—2018年5月48例风湿性疾病患者,根据其是否合并心血管疾病,分为研究组(n=24)和常规组(n=24),其中无心血管疾病合并患者,定义为常规组,合并心血管疾病患者,定义为研究组。分析25-羟-维生素D水平与心血管疾病的关系。结果研究组25-羟-维生素D水平为(7.53±2.74)ng·mL,常规组25-羟-维生素D水平为(18.63±4.62)ng·mL,研究组低于常规组(t=10.124,P=0.000);研究组内中膜厚度以及总胆固醇、颈动脉粥样硬化率等指标,与常规组比较,差异有统计学意义(t=4.416,P=0.000;t=3.494,P=0.001;χ^2=6.454,P=0.011);研究组骨密度值低于于常规组(t=2.615,P=0.012)。经过Logistic回归分析,内中膜厚度高水平以及25-羟-维生素D低水平,均为风湿性疾病患者发生心血管疾病的危险因素(P=0.028、0.001)。结论风湿性疾病25-羟-维生素D水平高低,能够影响心血管疾病的发生。 Objective To study the relationship between 25-hydroxy-vitamin D levels and cardiovascular disease in patients with rheumatic diseases.Methods 48 patients with rheumatic diseases from May 2017 to May 2018 were selected.According to whether they had cardiovascular disease,they were divided into study group(n=24)and routine group(n=24),including no cardiovascular disease.Patients were enrolled,defined as the regular group,with patients with cardiovascular disease,defined as the study group.The relationship between 25-hydroxy-vitamin D levels and cardiovascular disease was analyzed.Results The level of 25-hydroxy-vitamin D in the study group was(7.53±2.74)ng·mL,and the level of 25-hydroxy-vitamin D in the routine group was(18.63±4.62)ng·mL.The study group was lower than the conventional group(t=10.124,P=0.000).Intima-media thickness and total cholesterol,carotid atherosclerosis rate and other indicators were compared with the conventional group,and the difference was statistically significant(t=4.416,P=0.000;t=3.494,P=0.001;χ^2=6.454,P=0.011);the study group bone density values were lower than the conventional group(t=2.615,P=0.012).After logistic regression analysis,high intima-media thickness and low 25-hydroxy-vitamin D levels were risk factors for cardiovascular disease in patients with rheumatic diseases(P=0.028,0.001).Conclusion The level of 25-hydroxy-vitamin D in rheumatic diseases can affect the occurrence of cardiovascular diseases.
