摘要
目的探讨穴位埋线深度与神经根型颈椎病治疗疗效的关系。方法选取155例神经根型颈椎病患者为研究对象,按照治疗方法将患者分为假穴位埋线组(假埋线组,51例)、中层穴位埋线组(中层组,52例)和深层穴位埋线组(深层组,52例)。观察3组患者治疗疗效;观察3组患者治疗后颈部活动度情况;观察3组患者治疗前及治疗后1周、3周、5周视觉模拟评分(VAS)及YT-20评分变化情况;观察3组患者治疗前后生活质量情况及治疗后不良反应发生情况。结果深层组总有效率为94.2%,明显高于假埋线组的60.8%及中层组的78.8%(均P<0.05)。3组治疗后颈椎伸展度、颈椎屈曲度、颈椎旋转度、颈椎侧屈度均较治疗前升高(P<0.05),其中深层组明显高于假埋线组及中层组(P<0.05)。3组治疗后VAS评分均明显降低,但深层组VAS评分明显低于中层组及假埋线组(P<0.05)。3组治疗后YT-20评分均呈升高趋势(P<0.05),其中深层组YT-20评分高于中层组及假埋线组(P<0.05)。3组治疗后物理组成总分(PCS)评分及心理成分总分(MCS)评分均明显高于治疗前(P<0.05),其中深层组及中层组PCS评分、MCS评分均高于假埋线组(P<0.05)。3组均未发生严重不良反应。结论深层穴位埋线治疗神经根型颈椎病疗效较好,能缓解患者疼痛,促进患者颈功能恢复,改善患者生活质量,且不良反应少。
Objective To investigate the relationship between the depth of acupoint catgut embedding and its therapeutic effect on cervical spondylotic radiculopathy.Method One hundred and fifty-five patients with cervical spondylotic radiculopathy were enrolled as subjects and allocated to a sham acupoint catgut embedding(SACE,n=51),middle-layer acupoint catgut embedding(MLACE,n=52)and deep-layer acupoint catgut embedding(DLACE,n=52)groups according to therapeutic methods.In the three groups of patients,the therapeutic effects were evaluated;neck activity was examined after treatment;the Visual Analogue Scale(VAS)score and the YT-20 score were recorded before treatment and at one,three and five weeks after;the quality of life was assessed before and after treatment;adverse reactions were observed after treatment.Result The total efficacy rate was 94.2%in the DLACE group,which was significantly higher than 60.8%in the SACE group and 78.8%in the MLACE group(all P<0.05).After treatment,the degrees of cervical spine extension,flexion,rotation and lateral flexion increased in the three groups compared with before and the increasing range was significantly larger in the DLACE group than in the SACE and MLACE groups(P<0.05).After treatment,the VAS score decreased significantly in the three groups but was significantly lower in the DLACE group than in the MLACE and SACE groups(P<0.05).After treatment,the YT-20 score tended to increase in the three groups and the increasing range was significantly larger in the DLACE group than in the MLACE and SACE groups(P<0.05).After treatment,the physical component summary(PCS)score and the mental component summary(MCS)score increased significantly in the three groups compared with before and were higher in the DLACE and MLACE groups than in the SACE group(P<0.05).No obvious adverse reactions occurred in the three groups.Conclusion Deep-layer acupoint catgut embedding has a better therapeutic effect on cervical spondylotic radiculopathy.It can relieve the pain,help to restore neck function and improve the quality of life in the patients with few adverse reactions.
