非皮质类固醇联合氟米龙滴眼液治疗季节性过敏性结膜炎的临床研究

Clinical study on non-corticosteroid therapy combined with fluorometholone eye drops in treatment of patients with seasonal allergic conjunctivitis

目的观察非皮质类固醇联合氟米龙滴眼液治疗季节性过敏性结膜炎(SAC)的临床效果。方法采用前瞻性随机对照研究法,选择2017年1月至2018年12月简阳市人民医院收治的140例SAC患者(共254眼)作为研究对象,按照随机平行分组法将其随机分为两组:对照组(n=70,126眼)采用非皮质类固醇治疗方案(0.1%盐酸奥洛他定滴眼液),观察组(n=70,128眼)在对照组治疗方案基础上加用0.1%氟米龙滴眼液治疗,比较两组患者治疗3 d、7 d后的临床疗效,治疗前后的症状评分、体征评分、总评分,眼压和不良反应发生情况。结果观察组患者治疗3 d后的临床总有效率(81.3%)高于对照组(54.0%),观察组患者治疗7 d的临床总有效率(96.9%)高于对照组(81.7%),差异均具有统计学意义(P<0.05);观察组和对照组患者治疗前的症状评分、体征评分、总评分差异无统计学意义[(8.34±1.89)分vs.(8.22±1.83)分,(7.36±1.35)分vs.(7.29±1.27)分,(16.24±2.53)分vs.(15.98±2.65)分,P>0.05],但治疗7 d后的症状评分、体征评分、总评分差异均具有统计学意义[(2.04±0.75)分vs.(4.32±0.69)分,(1.96±0.63)分vs.(4.29±0.77)分,(4.26±0.89)分vs.(8.11±0.77)分,P<0.05];观察组和对照组患者治疗后的眼压比较差异无统计学意义[(16.22±1.62)mmHg vs.(15.93±1.53)mmHg,P>0.05];观察组和对照组患者在治疗期间均未见严重不良反应发生。结论非皮质类固醇治疗基础上联合应用氟米龙滴眼液治疗SAC的临床效果优于单纯非皮质类固醇给药治疗,起效时间更短,疗效更显著,且不良反应未显著增加,具有较好的临床应用价值。

Objective To observe the clinical effect of non-corticosteroid therapy combined with fluorometholone eye drops in treatment of patients with seasonal allergic conjunctivitis(SAC).Methods A prospective randomized controlled trial was conducted in selection of 140 patients(254 eyes)with seasonal allergic conjunctivitis admitted to the Jianyang People's Hospital during January 2017 to December 2018 as study subjects.They were randomly divided into two groups according to the random parallel grouping method:patients in control group(n=70,126 eyes)were treated with non-corticosteroid(0.1%olopatadine hydrochloride eye drops),and patients(n=70,128 eyes)in observation group were added 0.1%to that of control group.The efficacy of flumimetholone eye drops treatment in comparing with the therapeutic effect of patients in these two groups after 3 and 7 days of treatment including symptom scores,physical scores,total scores,intraocular pressure and adverse reactions before and after treatment.Results The total clinical effective rate(81.3%)in patients of observation group was higher than that(84.0%)of control group after treatment for 3 days.The total clinical effective rate(96.9%)of observation group was higher than that(81.7%)of control group.All of these were statistically significant(P<0.05).There was no significant difference in symptom scores,physical scores and total scores between observation group and control group[(8.34±1.89)points vs.(8.22±1.83)points,(7.36±1.35)points vs.(7.29±1.27)points,(16.24±2.53)points vs.(15.98±2.65)points,P>0.05],but the symptom scores,physical scores and total scores after 7 days of treatment have statistical significance[(2.04±0.75)points vs.(4.32±0.69)points,(1.96±0.63)points vs.(4.29±0.77)points,(4.26±0.89)points vs.(8.11±0.77)points,P<0.05].There was no significant difference in intraocular pressure between observation group and control group before and after treatment[(16.22±1.62)mmHg vs.(15.93±1.53)mmHg,P>0.05].Patients in observation group and control group were treated during the therapeutic course.There was no serious adverse reaction occurred.Conclusion On the basis of non-corticosteroid treatment,combined application of flumirone eye drops in treatment of patients with SAC have better clinical effect than that with single drug treatment,shorter onset time,more significant effect,and no significant increase in adverse reaction,hencce it has better clinical significance for application.
