骨水泥分布模式对I、II期Kümmell病经皮椎体后凸成形术疗效的影响

The effect of distribution pattern of bone cement on the outcome of percutaneous kyphoplasty for stage I and II Kümmell’s disease

目的 回顾性研究骨水泥分布模式对I、II期Kümmell病经皮椎体成形术疗效的影响。 方法 根据骨水泥在椎体内的分布形态,将85例接受经皮椎体成形术的Kümmell病患者分为A组(团块组)和B组(团块+嵌插组),比较两组患者骨水泥注入量、渗漏率、手术前与手术后、术后3月、6月、1年的VAS评分、ODI指数变化。 结果 B组患者骨水泥注入量显著多于A组,两组骨水泥渗漏率无明显差异,两组患者术后VAS及ODI评分较术前明显改善,但术后3月、6月、1年时B组VAS及ODI评分明显优于A组。 结论 PKP是治疗I、II期Kümmell病的有效方法,骨水泥分布模式是影响PKP术后疗效的重要因素,团块状+嵌插样骨水泥分布模式效果好于团块状分布。

Objective To retrospectively study the effect of bone cement distribution pattern on the outcome of percutaneous kyphoplasty (PKP) for stage I and II Kümmell's disease. Methods According to the distribution of bone cement in vertebral body, 85 patients with Kümmell's disease who underwent PKP were divided into group A (lump group) and group B (lump + insertion group). The volume of bone cement injection, leakage rate, VAS score, and ODI index before and 3 months, 6 months, and 1 year after operation were compared between the two groups. Results The amount of bone cement injected in group B was significantly more than that in group A. There was no significant difference in cement leakage rate between the two groups. The VAS and ODI scores of the two groups were significantly improved after operation, but the VAS and ODI scores in group B were significantly better than those in group A at 3 months, 6 months, and 1 year after operation. Conclusion PKP is an effective method for the treatment of stage I and II Kümmell's disease. The distribution pattern of bone cement is an important factor affecting the curative effect of PKP. The effect of the distribution pattern of mass + insertion bone cement is better than that of mass bone cement.
