加速康复外科护理在小儿人工耳蜗植入术中的应用

Application of Accelerated Rehabilitation Surgical Nursing In Pediatric Cochlear Implantation

目的探究加速康复外科护理在小儿人工耳蜗植入术中的应用效果。方法选取2017年12月-2018年12月收治我院的124例感音神经性耳聋患儿作为本次研究对象,按随机抽样法分为两组,对照组和观察组各62例。对照组62例,采用人工耳蜗植入术围手术期常规护理。观察组62例,在对照组的基础上,采用加速康复外科护理。比较两组患儿术后出现疼痛、睡眠障碍、便秘、敷料周围肿胀的发生率,术后脱水热、哭闹的发生率,术前、术后平均禁食水时间及术后患儿家属对护理的满意度。结果观察组患儿术后出现疼痛、睡眠障碍、便秘、敷料周围肿胀的发生率明显低于对照组,χ^2值分别为6.937、4.644、9.538、7.008,P<0.05,差异具有统计学意义;观察组患儿术后脱水热发生率9.68%、哭闹发生率33.87%,明显低于对照组27.42%的脱水热发生率、70.97%的哭闹发生率,χ^2值分别为6.459、17.105,P<0.05,差异具有统计学意义;术前、术后平均禁食水时间短于对照组,t值分别为18.55、34.08、33.86、34.60,P<0.05,差异具有统计学意义;患儿家属护理满意度观察组为98.39%,高于照组的83.87%,χ^2值为8.08,P<0.05,差异具有统计学意义。结论对小儿患者人工耳蜗植入术围手术期实行加速康复外科护理,能够显著减少术后不适的发生率,降低术后脱水热和哭闹的发生率,缩短术前、术后平均禁食水时间,提高患儿家长对护理服务的满意度。

Objective To report outcomes of accelerated rehabilitation surgical nursing following pediatric cochlear implantation. Methods Children(n=124) with sensorineural hearing loss admitted to our hospital from December2017 to December 2018 for cochlear implantation were included in this study. They were randomly divided into a control group and a study group(n= 62 each). Those in the control group received routine perioperative nursing, while those in the observation group received accelerated rehabilitation nursing. The incidence of postoperative pain, sleep disturbance, constipation, swelling around dressing, postoperative dehydration fever, crying, average time of fasting before and after surgery, and family satisfaction on postoperative nursing were compared between the two groups. Results The incidence of postoperative pain, sleep disturbance, constipation and dressing surrounding swelling in the study group was significantly lower than that in the control group(P < 0.05). The incidence for postoperative dehydration fever was9.68% and 33.87% for crying in the study group, lower than in the control group(27.42%). Mean time of fasting before and after surgery was also shorter in the study group than in the control group(P < 0.05), and family satisfaction in the study group(98.39%) higher than in the control group(83.87%)(P < 0.05). Conclusion In the perioperative period for pediatric cochlear implantation patients, accelerated rehabilitation nursing can significantly reduce the incidence of postoperative discomfort, dehydration fever and crying, shorten the preo-and postoperative fasting time, and improve family satisfaction with nursing.
