帕金森病患者外周血中颗粒酶B和穿孔素的表达及临床意义 被引量:1

摘要 目的检测帕金森病患者(PD)外周血中穿孔素(Pf)和颗粒酶B(Gz-B)的表达,探讨其临床意义。方法选择2018年12月-2019年6月在新疆医科大学相关附属医院就诊,并且符合帕金森病诊断标准的35例患者作为病例组,按其运动障碍分为震颤型PD(TD)和姿势平衡障碍型(PIGD)2个亚型,应用Hoehn-Yahr(H-Y)分级量表评估病情严重程度。选择门诊同期健康体检者35例为对照组,抽取外周血,采用三色流式细胞术胞内法,检测两组外周血标本中杀伤性T细胞(CTL)及自然杀伤细胞(NK)中Gz-B和Pf的表达,进一步分析CTL和NK细胞Gz-B和Pf表达与其病情严重程度及运动障碍分型的相关性。结果病例组患者外周血中CTL和NK细胞中Gz-B和Pf的表达水平均高于对照组(P<0.01)。病例组中震颤型与姿势平衡障碍型患者外周血中Gz-B和Pf表达差异无统计学意义。CTL细胞中Gz-B的表达与患者病情严重程度和病程时间呈正相关,差异具有统计学意义(P<0.05)。结论PD患者外周血CTL和NK细胞相关颗粒酶B及穿孔素的表达显著高于正常人外周血。 ObjectiveTo detect the expression of perforin(Pf)and granzyme B(Gz-B)in natural killer cell(NK),killer T cells(CTL)in peripheral blood from patients with Parkinson′s disease(PD),and to explore their possible clinical significance.Methods35 patients meeting the diagnostic criteria of Parkins′disease were selected as the PD group from December 2018 to June 2019 at the relevant affiliated hospitals of Xinjiang Medical University.The severity of disease was appraised by the Hoehn-Yahr(H-Y)classification scale according to the 2 subtypes of the tremor-type PD(TD)and the postural balance disorder(PIGD)following their dyskinesia.35 cases of healthy physical examination in the outpatients were selected as control group.Peripheral blood was extracted,and the expression of Gz-B and Pf in NK cells was detected by three-color flow cytometry.The relationship between the expression of Gz-B and Pf of CTL and NK cells and its severity and dyskinesia were further analyzed.ResultsThe expression level of Gz-B and PF in CTL and NK cells of PD group was shown to be significantly higher than that of control group(P<0.01).No significant difference can be observed of the expression of Gz-B and PF in peripheral blood between patients with tremor and those with postural balance disorder in PD group.The expression of Gz-B in CTL cells was positively and markedly correlated with the severity and duration of the disease(P<0.05).ConclusionThe expression of CTL and NK cell-associated Gz-B and Pf in peripheral blood of PD were significantly higher than that of normal peripheral blood.
