中药灌肠治疗晚期肝癌肠梗阻疗效观察

Clinical Efficacy of Traditional Chinese Medicine Enema on Intestinal Obstruction in Patients with Advanced Liver Cancer

目的探究中药灌肠治疗晚期肝癌肠梗阻的临床疗效。方法选取2018年3月至2019年3月河南中医药大学第三附属医院消化内科及河南省人民医院消化内科收治的72例晚期肝癌肠梗阻患者作为研究对象,并按照随机数表法将其随机分为试验组(36例)和对照组(36例),其中试验组患者采用中药灌肠治疗,对照组患者采用肥皂水灌肠治疗,对比两组患者的临床疗效及卡氏功能状态(karnofsky performance status,KPS)评分。结果治疗7 d后,试验组患者治疗总有效率为83.34%.明显高于对照组的治疗总有效率52.78%(Z=-2.720,Z=0.006);治疗7 d后,两组患者KPS评分均明显高于治疗前(t=19.395、2.226,P=0.000、0.029),且试验组患者KPS评分明显高于对照组(t=14.377,P=0.000)。结论中药灌肠治疗晚期肝癌肠梗阻,能够明显缓解患者腹痛、腹胀、恶心、呕吐等症状,改善患者排气、排便功能,提高患者生活质量,值得临床推广应用。

Objective To investigate the clinical efficacy of traditional Chinese medicine enema in the treatment of intestinal obstruction in patients with advanced liver cancer.Methods Seventy-two patients with advanced liver cancer and intestinal obstruction,admitted to the Department of Gastroenterology,Third Affiliated Hospital of Henan University of Traditional Chinese Medicine and the Department of Gastroenterolpgy,Henan Provincial People’s Hospital from March 2018 to March 2019,were divided,according to the random number table,into an experiment group(36 cases)and a control group(36 cases).The patients in the experiment group were treated with traditional Chinese medicine enema while the patients in the control group were treated with soapsuds enema.The clinical efficacy and Karnofsky performance status(KPS)score of the two groups were analyzed and compared.Results After 7 days of treatment,the total effective rate in the experiment group was 83.34%,which was significantly higher than 52.78%in the control group(Z=-2.720,P=0.006),and the KPS scores in the two groups were significantly higher than their respective levels before the treatment(t=19.395 and 2.226 respectively,P=0.000 and 0.029 respectively),and moreover,the KPS in the experiment group was significantly higher than that in the control group(t=14.377,P=0.000).Conclusion For the treatment of intestinal obstruction in patients with advanced liver cancer,traditional Chinese medicine enema can significantly relieve patients’symptoms,like abdominal pain,bloating,nausea and vomiting,boost intestinal gas exhaust and defecation,thereby improving the quality of life of patients,and deserving to be promoted in clinical practice.
