腮腺肌上皮瘤的CT影像分析

CT imaging analysis of parotid myoepithelloma

目的探讨腮腺肌上皮瘤的CT影像特征,以提高对本病的影像诊断水平。方法回顾性分析我院经手术、病理确诊为腮腺肌上皮瘤的17例患者的CT资料。17例中男性6例,女性11例,年龄32~60岁,中位年龄48岁,病程1~18个月。对肿瘤的部位、大小、形态、边界、CT平扫密度及三期强化特点进行分析。结果17例患者均为单发,左侧10例,右侧7例,9例密度均匀,8例密度不均匀,呈囊实性,其中腮腺浅叶8例,深叶6例,3例同时累及深浅叶,肿瘤直径均<3cm,增强扫描动脉期明显强化,静脉期及延迟期渐进性强化。结论腮腺肌上皮瘤具有一定的CT影像特点,结合发病年龄和临床资料可提高术前诊断准确率,最终确诊需依靠病理学。

Objective To explore the CT imaging features of parotid myoepithelioma in order to improve the imaging diagnosis of parotid myoepithelioma.Methods CT findings of 17 patients with myoepithelioma of parotid gland confirmed by surgery and pathology in our hospital were retrospectively analyzed.Those 17 patients included 6 man and 11 women,with a median age of 48 years old.The tumor locatioin,size,morphology,edge,CT density,and enhancement characteristics were all ananlyzed in this work.Results Each of those 17 patients had only 1 tumor.10 were on the left side,7 on the right side,9 were homogeneous in density and 8 were cystic and solid in density.8 lesions were located in the superficial lobe,6 lesions in the deep lobe and 3 lesions across the deep and uperficial lobe.The diameter of tumors was less than 3 cm.The arterial phase and the venous phase and the delayed phase were enhanced gradually.Conclusion Myoepithelial tumor of parotid gland has certain CT imaging characteristics.Combined with the age of onset and clinical data,the accuracy of preoperative diagnosis can be improved.However,the final diagnosis should be depended on the histopathological and immunohistochemical results.
