免疫细胞化学技术和免疫组织化学技术在甲状腺乳头状癌中的比较研究 被引量:1

摘要 目的探讨应用免疫细胞化学(immunocytochemistry,ICC)技术检测甲状腺细针穿刺细胞学(fine needle aspiration cytology,FNAC)标本中CK19、MC及TPO表达情况,并将结果与免疫组织化学(immunochemistry,IHC)进行比较。方法收集85例甲状腺乳头状癌(papillary thyroid carcinoma,PTC)患者的FNAC标本和相应组织学标本,采用EnVision法检测CK19、MC及TPO在细胞学和组织学中的表达情况。结果在ICC检测结果中,CK19、MC及TPO分别有81例、76例、7例阳性;在IHC检测结果中,CK19、MC及TPO分别有83例、79例、8例阳性。对于CK19,ICC与IHC一致性为97.65%,敏感性为97.59%,特异性为100%,阳性预测值(PPV)和阴性预测值(NPV)为100%和50%。对于MC,ICC与IHC一致性为94.12%,敏感性为94.94%,特异性为83.33%;PPV和NPV为98.68%和55.56%。对于TPO,ICC与IHC一致性为96.47%,敏感性为98.70%,特异性为75%;PPV和NPV为97.44%和85.71%。结论ICC技术在PTC的诊断结果与IHC技术相当,可以作为诊断PTC重要辅助手段。 Objective To investigate the expression of CK19,MC and TPO in the specimens of Fine needle aspiration cytology(FNAC)by immunocytochemistry(ICC),and compare the immunocytochemistry results with immunohistochemistry(IHC)results.Methods The FNAC and corresponding histological specimen of 85 patients with papillary thyroid carcinoma(PTC)were collected.The expression of CK19,MC and TPO in cytology and histology were detected by EnVision method.Results Among the result of ICC test,81 cases,76 cases and 7 cases were positive for CK19,MC and TPO respectively.Among the IHC test results,83 cases,79 cases and 8 cases were positive for CK19,MC and TPO,respectively.For CK19,ICC and IHC had a consistency of 97.65%,a sensitivity of 97.59%,a specificity of 100%,a PPV of 100%and a NPV of 50%.For MC,ICC and IHC had a consistency of 94.12%,a sensitivity of 94.94%,a specificity of 83.33%,a PPV of 98.68% and a NPV of 55.56%.For TPO,ICC and IHC had a consistency of 96.47%,a sensitivity of 98.70%,a specificity of 75%,a PPV of 97.44% and a NPV of 85.71%.Conclusion The results of ICC technology in the diagnosis of PTC are comparable to those of IHC technology and can be used as an important assistant test for diagnosing PTC.
