全乳晕入路腔镜甲状腺在结节性甲状腺肿中的应用价值

The Value of Endoscopic Thyroidectomy in Nodular Goiter Via Full Areola Approach

目的探究经全乳晕入路腔镜甲状腺手术治疗方法在结节性甲状腺肿患者手术临床治疗中的应用价值。方法选取2016年8月-2018年12月,在医院中进行结节性甲状腺肿治疗的患者共78例,均为女性患者。采用掷骰子分组法,将掷出奇数的患者纳入到对照组,给予开放手术治疗方法。将掷出偶数的患者纳入到观察组,给予经全乳晕入路腔镜甲状腺手术治疗方法。观察两组患者的出血量、手术时间、住院时间、住院费用、术后引流量等手术指标;观察两组患者的感染、渗血、吞咽不适、喉头水肿、声音嘶哑、皮下气肿术后并发症发生率。结果观察组患者的出血量、住院时间、术后引流量低于对照组,手术时间、住院费用高于对照组,差异具有统计学意义(P<0.01)。观察组术后并发症发生率为10.26%,低于对照组的28.21%,差异具有统计学意义(P<0.05)。结论将经全乳晕入路腔镜甲状腺手术治疗方法应用到结节性甲状腺肿患者治疗中,降低了术后并发症发生率,取得了良好的临床手术效果。

Objective To investigate the clinical value of total breast surgery in patients with nodular goiter. Methods From August 2016 to December 2018, 78 patients were treated for nodular goiter in hospital, all female patients. An odd number of patients were included in the control group using the rolling die grouping method and an open surgical treatment was given. An even number of patients were included in the observation group and treated with an all-lacto-access endoscopic thyroid surgery. Observe the bleeding volume, operation time, stay time, hospitalization cost. The postoperative drainage volume and other surgical indexes were observed, and the incidence of postoperative complications such as infection, bleeding, dysphagia discomfort, larynx edema, hoarseness and subcutaneous emphysema were observed. Results The amount of bleeding, hospitalization time and postoperative drainage in the observation group were lower than those in the control group, and the operation time and hospitalization cost were significantly higher than those in the control group(P<0.01). The incidence of postoperative complications in the observation group was 10.26%, which was significantly lower than that in the control group(28.21%, P<0.05). Conclusion Endoscopic thyroidectomy via whole areola approach was applied to the treatment of nodular goiter, which reduced the incidence of postoperative complications and achieved good clinical results.
