高渗氯化钠羟乙基淀粉40注射液对非体外冠脉移植手术中血管外肺水的影响 被引量:1

摘要 目的:探讨高渗氯化钠羟乙基淀粉40注射液(HSH40)对非体外循环冠脉旁路移植手术中血管外肺水的影响。方法:选取40例拟行非体外循环冠脉旁路移植的患者,随机分为观察组和对照组各20例,麻醉诱导气管插管后经股动脉穿刺置入PICCO,经颈内静脉穿刺置入中心静脉导管,观察组于麻醉诱导后输入4.2%氯化钠羟乙基淀粉4 ml/kg,20 min滴入;对照组于相同时段输入0.9%生理盐水4 ml/kg,20 min滴入,常规补胶体液6 ml/(kg·h)。术中间断静脉注射咪唑安定、舒芬太尼、阿曲库胺,持续泵注右美托咪啶、异丙酚及间断吸入0.5%~3%七氟烷维持麻醉。比较分析高渗液体输入后5 min(T 0)、输入后1 h(T 1)、输入后2 h(T 2)、输入后3 h(T 3)、输入后4 h(T 4)血管外肺水、肺血管通透指数、胸腔内血容量、心指数。结果:所有入组患者均顺利完成手术,H组的EVLW I与N组对比,差异无统计学意义(P>0.05),H组T 1时的CI显著高于本组T 0、T 3、T 4,且N组T 1时的CI显著低于H组T 1,H组T 1时的ITBV显著高于本组T 0、T 3、T 4,且N组T 1时的ITBV显著低于H组T 1,差异有统计学意义(P<0.05)。结论:高渗氯化钠羟乙基淀粉40注射液在容量治疗中具有不增加EVLW的作用,且有良好的扩容效果。
