粪便隐血试验序贯结肠镜检查在社区高危人群结直肠癌筛查中的应用价值

Application value of sequential examination of fecal occult blood test and colonoscopy for colorectal cancer screening in community high-risk residents

目的探讨粪便隐血试验序贯结肠镜检查在社区高危人群结直肠筛查中的应用价值。方法纳入两个社区存在高危因素的1 200例居民,均在行粪便隐血试验后接受结肠镜检查。以病理检查结果为金标准,评价粪便隐血试验序贯结肠镜检查对直肠癌的诊断效能。结果经粪便隐血试验联合结肠镜序贯检查,60例(5.00%)诊断为结直肠癌;根据病理检查,65例(5.42%)确诊为结直肠癌。粪便隐血试验联合结肠镜序贯检查诊断结直肠癌的灵敏度为89.23%(58/65),特异度为99.82%(1 133/1 135),准确度为99.25%(1 191/1 200),阳性预测值为96.7%(58/60),阴性预测值为99.39%(1 133/1 140)。结论在社区高危人群中粪便隐血试验序贯结肠镜检查对结直肠癌具有较好的筛查效果。

Objective To explore the application value of sequential examination of fecal occult blood test and colonoscopy for colorectal cancer screening in community high-risk residents.Methods A total of 1200 residents with high-risk factors were selected from two communities,and were received colonoscopy after fecal occult blood test.With the pathological examination results as the gold standard,the diagnostic efficacy of sequential examination of fecal occult blood test and colonoscopy for rectal cancer was assessed.Results Sixty(5.00%) cases were diagnosed as colorectal cancer by the sequential examination of fecal occult blood test and colonoscopy;65(5.42%) cases were diagnosed as colorectal cancer according to the pathological examination.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the sequential examination of fecal occult blood test combined with colonoscopy were 89.23%(58/65),99.82%(1 133/1 135),99.25%(1 191/1 200),96.7%(58/60) and 99.39%(1 133/1 140) respectively,for diagnosing colorectal cancer.Conclusion The sequential examination of fecal occult blood test and colonoscopy exerts a preferable effect on colorectal cancer screening in community high-risk residents.
