胫前减张切口在老年胫腓骨远端骨折锁定加压钢板治疗中的应用效果观察

Effects of anterior tibial reduced-tension incision in elderly patients with distal tibiofibular fractures treated by locking compression plate

目的:探讨胫前减张切口在老年胫腓骨远端骨折锁定加压钢板治疗中的应用效果。方法:选取62例胫腓骨远端骨折老年患者为研究对象,按照随机数字表法分为对照组与观察组各31例,对照组行锁定加压钢板内固定治疗,观察组在对照组的基础上结合胫前减张切口治疗,比较两组治疗效果。结果:观察组切口愈合时间及住院时间均显著短于对照组,差异有统计学意义(P<0.05),两组手术时间、术中出血量及术后引流量比较,差异无统计学意义(P>0.05)。术后,观察组踝关节功能优良率明显高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为22.6%,显著低于对照组的45.2%,差异有统计学意义(P<0.05)。结论:胫前减张切口在老年胫腓骨远端骨折锁定加压钢板治疗中的应用效果优于单纯锁定加压钢板内固定治疗效果。

Objective:To investigate effects of anterior tibial reduced-tension incision in elderly patients with distal tibiofibular fractures treated by locking compression plate.Methods:A total of 62 patients with distal tibiofibular fractures were selected as the study subjects.According to the random number table method,they were divided into control group(n=31)and observation group(n=31).The control group was treated with locking compression plate internal fixation,while the observation group was combined with the anterior tibial reduced-tension incision on the basis of that of the control group.The therapeutic effects of the two groups were compared.Results:The healing time and hospitalization time of the observation group were significantly shorter than those of the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the operation time,intraoperative blood loss and postoperative drainage between the two groups(P>0.05).After the operation,the excellent and good rate of ankle joint function in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 22.6%,which was significantly lower than that in the control group(45.2%),and the difference was statistically significant(P<0.05).Conclusions:Anterior tibial reduced-tension incision combined with locking compression plate internal fixation is superior to single locking compression plate internal fixation in the treatment of the elderly patients with distal tibiofibular fractures.
