彩色多普勒超声在间质部妊娠与宫角部妊娠鉴别诊断中的应用效果

Application effects of color Doppler ultrasound in differential diagnosis of interstitial pregnancy and uterine cornual pregnancy

目的:观察彩色多普勒超声在间质部妊娠与宫角部妊娠鉴别诊断中的应用效果的鉴别诊断价值。方法:选取收治的164例异位妊娠患者为研究对象,对其进行彩色多普勒超声检查,比较间质部妊娠与宫角部妊娠的不同分型声像图特征。结果:以临床病理学检验结果为金标准,其中90例患者为间质部妊娠,74例患者为宫角部妊娠。根据超声检查的声像图特征,诊断输卵管间质部妊娠93例,准确率为96.77%;诊断宫角部妊娠71例,准确率为95.95%。间质部妊娠与宫角部妊娠根据声像图特征可分为妊娠囊型与包块型。妊娠囊型共113例,其中妊娠囊位于宫角部的患者共60例,妊娠囊位于间质部的患者有53例;包块型共51例,经手术及临床病理证实宫角部妊娠30例,间质部妊娠21例。结论:彩色多普勒超声检查在间质部妊娠与宫角部妊娠除包块型外的鉴别诊断应用中准确率较高。

Objective:To investigate application effects of color Doppler ultrasound in differential diagnosis of interstitial pregnancy and uterine cornual pregnancy.Methods:164 patients with ectopic pregnancy admitted to our hospital were enrolled in the study.Color Doppler ultrasonography was performed to compare the different types of sonographic features of interstitial pregnancy and uterine cornual pregnancy.Results:The clinical pathological examination results were used as the gold standard,90 patients were diagnosed as interstitial pregnancy,and 74 patients were diagnosed as uterine cornual pregnancy.According to the sonographic features of ultrasonography,93 cases were diagnosed as interstitial pregnancy with an accuracy of 96.77%,and 71 cases were diagnosed as uterine cornual pregnancy with the accuracy of 95.95%.Interstitial pregnancy and uterine cornual pregnancy can be classified into gestational sac type and mass type according to the sonographic features.There were 113 cases of gestational type,including 60 cases of gestational sac located in the uterine horn and 53 cases of gestational sac located in the interstitial portion.There were 51 cases of mass type.Further,30 cases of uterine cornual pregnancy and 21 cases of interstitial pregnancy were confirmed by surgery and clinical pathological examination.Conclusions:Color Doppler ultrasonography has a higher accuracy in the differential diagnosis of interstitial pregnancy and uterine horn pregnancy except for the mass type.
