经气管镜超声引导活检钳活检术在纵隔和肺门病变诊断中的初步应用

Preliminary Application of Endobronchial Ultrasound-guided Miniforceps Biopsy in the Diagnosis of Mediastinal and Hilar Lesions

目的 探讨经气管镜超声引导小型活检钳活检(endobronchial ultrasound-guided miniforceps biopsy,EBUS-MFB)对纵隔和肺门病变进行活检的可行性及安全性。方法 2018年1月~2019年1月,对36例纵隔或肺门病变先后实施超声内镜引导下经支气管针吸活检(endobronchial ultrasound-guided transbronchial needle aspiration,EBUS-TBNA)和EBUS-MFB,获得组织标本送检,比较两者的检出率和并发症发生率。结果 EBUS-MFB组总体检出率为97%(35/36),EBUS-TBNA组为78%(28/36),差异有显著性(McNemar检验,P=0. 016),2组均无明显并发症发生。结论 EBUS-MFB对纵隔和肺门病变进行活检安全可行。

Objective To evaluate the efficacy and safety of endobronchial ultrasound-guided miniforceps biopsy(EBUS-MFB)for diagnosing mediastinal and hilar abnormalities.Methods A total of 36 patients with mediastinal or hilar lymphadenopathy between January 2018 and January 2019 were included in this study.Endobronchial ultrasound-guided transbronchial needle aspiration(EBUS-TBNA)and EBUS-MFB were performed separately,and specimens were acquired from each patient for histologic diagnosis.The diagnostic yield and incidence of complications of EBUS-TBNA and EBUS-MFB were compared.Results The overall diagnostic yield of EBUS-TBNA and EBUS-MFB was 78%(28/36)and 97%(35/36),respectively(McNemar’s test,P=0.016).No complications were encountered.Conclusion Endobronchial ultrasound-guided miniforceps biopsy can be used to obtain tissue specimens safely and effectively in mediastinal and hilar lesions with high diagnostic rates.
