肥胖与非肥胖高脂血症病人中医证型特点分析

Characteristics of TCM Syndromes in Obese and Nonobese Patients with Dyslipidemia

目的分析肥胖与非肥胖高脂血症病人中医证型的分布规律,为中医辨证论治高脂血症提供思路。方法选取2015年3月6日—2016年1月19日北京中医药大学东直门医院收治的高脂血症病人160例,对血脂异常病人进行流行病学调查和四诊信息收集,探讨肥胖与非肥胖血脂异常病人中医证型分布规律及其差异。按体质指数(BMI)水平进行分组,其中BMI≥25 kg/m^2为肥胖组,BMI<25 kg/m^2为非肥胖组。结果中医证型分布,肥胖组:痰浊阻遏>肝肾阴虚>脾肾阳虚>阴虚阳亢>气滞血瘀;非肥胖组:肝肾阴虚>脾肾阳虚>阴虚阳亢>痰浊阻遏>气滞血瘀。两组病人痰浊阻遏、脾肾阳虚、肝肾阴虚、阴虚阳亢所占百分率比较差异有统计学意义(P<0.05)。结论高脂血症病人中,肥胖病人以痰浊阻遏实证为主,而非肥胖病人以肝肾阴虚、脾肾阳虚、阴虚阳亢虚损型证型为主。

Objective To analysis characteristics of TCM syndromes in obese and nonobese patients with dyslipidemia,so as to provide ideas for treatment of hyperlipidemia.Methods One hundred and sixty patients with dyslipidemia in Dongzhimen Hospital,Beijing University of Chinese Medicine were selected,from March 6,2015 to January 19,2016.An epidemiological investigation was conducted,and the items of four diagnostic data were extracted.To explore characteristics of TCM syndromes in obese and nonobese patients with dyslipidemia.The patients were grouped according to BMI:BMI≥25 kg/m^2 as the obese group,BMI<25 kg/m^2 as the Nonobese group.Results The results showed that obese group:phlegm turbidity repression>liver kidney Yin deficiency>spleen kidney Yang deficiency>Yin deficiency Yang Hyperactivity>Qi stagnation and blood stasis;nonobese group:liver and kidney Yin deficiency>spleen kidney Yang deficiency>Yin deficiency Yang Hyperactivity>phlegm turbidity obstruction>Qi stagnation and blood stasis.There were significant diffierences in the percentage of phlegm turbidity repression,spleen kidney Yang deficiency,liver and kidney Yin deficiency,and Yin deficiency Yang Hyperactivity between the two groups.Conclusion Among the patients with hyperlipidemia,the obese patients were mainly diagnosed with phlegm turbidity obstruction,the nonobese patients were mainly diagnosed with liver and kidney Yin deficiency,spleen kidney Yang deficiency,Yin deficiency Yang.
