养心氏片对慢性心力衰竭病人焦虑、抑郁及生活质量的影响

Influence of Yangxinshi Tablet on Anxiety,Depression and Quality of Life in Patients with Chronic Heart Failure

目的观察养心氏对慢性心力衰竭病人焦虑、抑郁状态及生活质量的影响。方法选取2017年10月—2018年4月因慢性心力衰竭急性加重于大连大学附属中山医院心内科住院,病情平稳2~4周的病人80例,随机分为对照组(30例)、养心氏组(30例)、曲美他嗪组(10例)、养心氏+曲美他嗪组(10例)。分别采用焦虑自评量表(SAS)、抑郁自评量表(SDS)评估焦虑与抑郁状态,采用明尼苏达心力衰竭生活质量量表(MLHFQ)评价生活质量,采用6 min步行试验(6-MWT)评价病人运动耐量。结果6个月随访时,养心氏组、曲美他嗪组、养心氏+曲美他嗪组SAS、SDS评分均较治疗前明显降低(P<0.05),养心氏组、养心氏+曲美他嗪组治疗后SAS、SDS评分均较对照组明显降低(P<0.05)。养心氏组、曲美他嗪组、养心氏+曲美他嗪组治疗后6 min步行距离均较治疗前明显增加(P<0.05),且均较对照组治疗后明显增加(P<0.05)。4组治疗后MLHFQ评分均较治疗前明显降低(P<0.05),养心氏组、曲美他嗪组、养心氏+曲美他嗪组治疗后MLHFQ评分均较对照组明显降低(P<0.05)。结论在优化药物治疗基础上,口服养心氏片可以改善慢性心力衰竭病人的焦虑、抑郁状态,增加运动耐量,改善生活质量。

Objective To explore the effect of Yangxinshi tablet on anxiety,depression,and quality of life of patients with chronic heart failure(CHF).Methods Eighty patients with acute exacerbation of chronic heart failure in a stable condition for 2 to 4 weeks admitted to the Department of Cardiology,Zhongshan Hospital Affiliated to Dalian University from October 2017 to April 2018 were randomly divided into control group(n=30),Yangxinshi group(n=30),Trimetazidine group(n=10),Yangxinshi and Trimetazidine group(n=10).Self-rating Anxiety Scale(SAS)and self-rating Depression Scale(SDS)were used to assess anxiety and depression,Minnesota living with heart failure questionnaire(MLHFQ)was used to evaluate quality of life,and 6-minute walking test(6-MWT)was used to evaluate exercise tolerance.Results At 6 months follow-up,the SAS and SDS scores of Yangxinshi group,Trimetazidine group,and Yangxinshi and Trimetazidine group were significantly improved compared with those before treatment(P<0.05).The SAS and SDS scores of Yangxinshi group,Yangxinshi and Trimetazidine group were significantly decreased compared with those before treatment(P<0.05).The walking distance of Yangxinshi group,Trimetazidine group,and Yangxinshi and Trimetazidine group after treatment was significantly increased compared with those before treatment(P<0.05),and significantly prolonged compared with that of the control group after treatment(P<0.05).The MLHFQ scores of the four groups were significantly lower than those before treatment(P<0.05).The MLHFQ scores of Yangxinshi group,Trimetazidine group,and Yangxinshi and Trimetazidine group were significantly lower than that of the control group after treatment(P<0.05).Conclusion Yangxinshi tablet can improve anxiety and depression of patients with chronic heart failure,increase exercise tolerance,and improve the quality of life.
