摘要
目的:分析肺腺癌患者的表皮生长因子受体(EGFR)基因突变类型及不同性别、年龄、转移患者突变差异。方法:收集武汉大学人民医院2016-01-2017-03期间经病理确诊的肺腺癌患者230例,利用突变扩增阻滞系统联合肽核酸(ARMS-PNA)结合荧光PCR技术检测患者石蜡包埋病理组织样本中EGFR基因18-21外显子上的6种EGFR突变,同时分析不同性别、年龄、转移胰腺癌患者EGFR基因突变的差异。结果:肺腺癌患者EGFR突变率为53.58%,主要突变类型为19del(56.10%)和L858R(34.15%);男性肺腺癌患者的突变率显著低于女性肺腺癌患者(43.08%vs 65.42%,P<0.01);<60岁的EGFR突变型患者中,19del类型的突变比例显著高于≥60岁者(72.73%vs 42.65%,P<0.01),而L858R类型的突变比例显著低于≥60岁者(23.64%vs 42.65%,P<0.05);EGFR突变与否与肺腺癌转移无显著相关性(P>0.05)。结论:EGFR突变率与肺腺癌患者性别相关,而突变类型与年龄相关。
Objective:To investigate the relationship between the epidermal growth factor receptor(EGFR gene mutation and the sex,age and metastasis in patients with lung adenocarcinoma.Method:230 patients with pathologically proved adenocarcinoma of the lung were collected from the Renmin Hospital of Wuhan University from January 2016 to March 2017.Combined amplification refractory mutation system-peptide nucleic acids(ARMS-PNA)and fluorescence PCR technique,6 EGFR mutations in exon 18~21 of EGFR gene were detected in paraffin embedded tissue samples from patients.And then,the relationship between EGFR mutation status,gender,age,and metastasis was analyzed.Results:The mutation rate of EGFR in lung adenocarcinoma patients was 53.58%,and the main mutation types were 19 del(56.10%)and L858 R(34.15%).The mutation rate in men with lung adenocarcinomawas significantly lower than that in women with lung adenocarcinoma(43.08% vs 65.42%,P<0.01).In the EGFR mutant patients of low age group(<60 y),the mutation rates of type 19 del was significantly higher than that in the high age group(>60 y)(72.73% vs 42.65%,P<0.01),and the mutation rate of type L858 Rwas significantly lower than that in the high age group(23.64%vs 42.65%,P<0.05).Finally,there is no significant correlation between the mutation of EGFR and the metastasis of lung adenocarcinoma(P>0.05).Conclusion:The EGFR mutation rate is associated with gender in lung adenocarcinoma,while the mutation type is associated with age.
