尪痹“从脾论治”学术思想探讨

Discussion on the Academic Thought of “Treating from Spleen” of Wang bi

文章旨在探讨从脾脏辨证论治尪痹早期的中医理论思想。以名老中医沈冯君教授的理论为基调,以"脾为后天之本,气血生化之源"为核心,从正邪两方面入手辨证,正虚为营卫运行失常、气血亏虚,邪盛则为痰瘀内阻、湿痰阻络。通过从脾论治,脾气健运,气机升降和合,使气血生化有源,以此扶正祛邪,濡养筋骨关节。以往医家多补益肝肾,以治其先天禀赋不足所致的正虚邪盛为主,而在沈冯君教授理论指导下,亦倡导健脾之法,以后天补益先天,化生气血,通利关节,达到缓解尪痹症状作用。

The core idea of this paper is to explore the theory of Traditional Chinese Medicine from the spleen to treat early Rheumatoid Arthritis. Based on the theory of Shen Fengjun, the old and famous doctor of our university, taking "the spleen is the acquired source of human growth and development, it is the source of the development of Qi and Blood transport" as the core. Starting from the two aspects of good and evil with the purpose of syndrome differentiation, weakened body resistance includes the abnormal operation of Ying-wei and deficiency of Qi and Blood,and Evil stagnation includes stagnations of phlegm-stasis and Phlegm-dampness Retention. By treating from Spleen, we can strengthen Spleen and benefit vital Qi turned to be normal about rising and descending, supple Qi-Blood. In this way, we can strengthen vital Qi and dispel pathogens, and nourish the bones and joints. In the past, doctors used to recuperate the liver and kidney to treat diseases caused by congenital deficiency of the human body. Under the guidance of Shen Fengjun’s theory, this paper advocates the method of invigorating spleen aim at making up for the shortcomings of human body, recuperating people’s Qi and nourishing blood and joints. Finally, we can alleviate the symptoms of Rheumatoid Arthritis as a whole.
