RAAS主要标志物在心房颤动中的临床价值研究

目的探讨肾素-血管紧张素-醛固酮系统(RAAS)主要标志物在心房颤动中的临床诊断及应用价值。方法分别收集健康对照组100例和行导管消融治疗的心房颤动患者108例;收集所有研究对象的一般临床资料和氨基端脑钠肽前体(NT-proBNP)检测结果,并用化学发光仪测定房颤患者术前及术后、对照组卧位血浆血管紧张素(AngⅡ)、肾素活性(PRA)、醛固酮(ALD)、皮质醇水平。结果心房颤动组与健康对照组进行对比,其中NT-proBNP、LAD差异有统计学意义(P<0.05);心房颤动组术前血浆PRA、AngⅡ、ALD、cortisol水平与对照组比较差异有统计学意义(P<0.05),术后血浆水平与对照组比较,同样差异有统计学意义(P<0.05);心房颤动组术后血浆PRA、AngⅡ、ALD、cortisol水平明显升高,与术前相比,差异有统计学意义(P<0.05)。结论RAAS主要标志物PRA、AngⅡ、ALD血浆水平可用于房颤的临床常规检查及诊断。

Objective To investigate the clinical diagnostic value of the main markers of renin-angiotensin-aldosterone system(RAAS)in atrial fibrillation.Methods 100 cases of atrial fibrillation in healthly control group and 108 cases of atrial fibrillation treated by catheter ablation were collected,and the general clinical data of all subjects were collected.Angiotensin II,plasma rennin activity(PRA)and aldosterone(ALD)were measured by chemiluminescence before and after operation and in the control group cortisol level.Results Compared with the normal control group,the levels of N-terminal precursor protein of brain naturally occurring peptide(NT-proBNP)and LAD in atrial fibrillation group were significantly different(P<0.05);the levels of PRA,Ang II,ALD and cortisol in plasma before operation in atrial fibrillation group were significantly different from those in control group(P<0.05),and the levels of plasma after operation were significantly different from those in control group(P<0.05).The levels of PRA,Ang II,ALD and cortisol in patients with atrial fibrillation were significantly higher than those before operation(P<0.05).Conclusion The plasma levels of PRA,Ang II and ALD,the main markers of RAAS,can be used for routine clinical examination and diagnosis of atrial fibrillation.
