超选择子宫动脉栓塞的疗效及对卵巢内分泌功能的影响

Effect of Superselective Uterine Artery Embolization on Ovarian Endocrine Function

目的:探讨超选择子宫动脉栓塞治疗子宫肌瘤的临床疗效及卵巢功能的变化。方法:选取2016年5月-2017年12月期间本院收治的子宫肌瘤患者78例,随机分为对照组和实验组,对照组行开腹子宫肌瘤剔除术治疗,实验组采取超选择子宫动脉栓塞治疗方案,比较两组治疗手段对卵巢内分泌功能的影响。结果:观察组总有效率为92.3%,对照组为97.4%,组间比较差异不存在统计学意义(P>0.05);术后实验组E2、LH、FSH水平无明显变化(P>0.05),而对照组的E2水平明显下降, LH、FSH水平显著升高(P<0.05),且实验组的E2、LH、FSH水平与对照组比较差异显著(P<0.05);术后随访12个月内,对照组子宫肌瘤残留率和复发率与实验组比较差异均无统计学意义(P>0.05)。结论:超选择子宫动脉栓塞治疗子宫肌瘤,创伤小且临床疗效确切,对保护患者卵巢内分泌功能有较高价值。

Objective:To explore the clinical value of superselective uterine artery embolization in the treatment of uterine leiomyoma.Methods:78 patients with uterine leiomyoma admitted to our hospital from May 2016 to December 2017,who were randomly divided into two groups. The control group was treated with open myomectomy, while the observation group was treated with superselective uterine artery embolization. The therapeutic effects of the two groups were compared.Results:The total effective rate was 92.3% in the observation group and 97.4% in the control group. There was no significant difference between the two groups(P>0.05). The levels of E2, LH and FSH in the observation group did not change significantly after operation(P>0.05), while the levels of E2 in the control group decreased significantly and the levels of LH and FSH increased significantly(P<0.05). The levels of E2, LH and FSH in the observation group were significantly different from those in the control group(P<0.05). After 12 months of follow-up, there was no significant difference in the residual rate and recurrence rate of uterine leiomyoma between the control group and the observation group(P>0.05).Conclusion:Super-selective uterine artery embolization for uterine leiomyoma has a small trauma and a definite clinical effect. It has a high value in protecting ovarian endocrine function of patients.
