基于Logistic回归分析的ICU人工气道干预患者医院获得性肺炎危险因素调查

Investigation of Risk Factors for Hospital Acquired Pneumonia in ICU Artificial Airway Intervention Patients based on Logistic Regression Analysis

目的探讨我院ICU人工气道干预患者医院获得性肺炎(HAP)发生现状及其危险因素。方法选取2017年1月~2019年1月我院收治的ICU人工气道干预患者150例,经临床诊断、影像学检查及病理学诊断确定发生HAP的患者例数,借助单因素、多因素Logistic回归分析明确HAP危险因素。结果经临床诊断、影像学检查及病理学诊断确定HAP患者25例,发生率16.67%;经单因素、多因素Logistic回归分析显示,ICU人工气道干预患者HAP危险因素有年龄、基础疾病≥3种、人工气道干预≥5d、胃管留置时间≥2d、抗生素使用、制酸剂使用(P<0.05)。结论我院ICU人工气道干预患者HAP发生率为16.67%,且年龄>55岁、基础疾病≥3种、人工气道干预≥5d、胃管留置时间≥2d、抗生素使用、制酸剂使用是ICU人工气道干预患者HAP危险因素。

Objective To investigate the current status and risk factors of hospital acquired pneumonia in patients with artificial airway intervention in ICU.Methods 150 cases of ICU artificial airway intervention patients admitted to our hospital from January 2017 to January 2019 were selected.The number of patients with hospital acquired pneumonia was determined by clinical diagnosis,imaging examination and pathological diagnosis.Factors and multivariate logistic regression analysis were used to identify risk factors for hospital acquired pneumonia.Results A total of 25 patients with hospital acquired pneumonia were diagnosed by clinical diagnosis,imaging examination and pathological diagnosis,the incidence rate was 16.67%.ICU artificial airway intervention patients with hospital acquired pneumonia were screened by single factor and multivariate logistic regression analysis.Risk factors were age>55 years,more than 3 basic diseases,artificial airway intervention≥5d,gastric tube indwelling≥2d,antibiotic use,antacid application(P<0.05).Conclusion The incidence of hospital acquired pneumonia in ICU artificial airway intervention patients in our hospital was 16.67%,and the age>55 years,more than 3 basic diseases,artificial airway intervention≥5d,indwelling gastric tube≥2d.Antibiotic use and antacid application were risk factor for hospital acquired pneumonia in ICU artificial airway intervention patients.
