不同年龄段结直肠癌患者的临床特点及病理分析研究

Clinical characteristics and pathological analysis of colorectal cancer in different age groups

目的探究不同年龄段结直肠癌患者临床特点及病理特点.方法选取2016年11月至2018年3月本院收治的结直肠癌患者60例为研究对象,根据患者不同年龄段分为青中年组和老年组,每组30例.青中年组为年龄在60岁以下的结直肠癌患者,老年组为年龄超过60岁的结直肠癌患者.分析两组患者临床病理特点.结果两组患者在手术时间、手术方式、性别、患病时间、住院时间及术中出血量方面比较差异无统计学意义;两组患者在肿瘤位置方面比较差异有统计学意义(P<0.05);青中年组与老年组A期差异无统计学意义;在B期、C期、D期方面,青中年组与老年组比较差异有统计学意义(P<0.05).在病理学形态、原发肿瘤浸润深度及组织学类型方面比较差异有统计学意义(P<0.05).结论在临床症状方面,老年结直肠癌与青中年直肠癌比较差异无统计学意义,所以需要根据结直肠镜、形状及患者饮食等习惯,及早诊断老年及中青年结直肠癌,改善患者预后效果.

Objective To explore the clinical and pathological characteristics of colorectal cancer patients of different ages. Methods Sixty pa- tients with colorectal cancer admitted to our hospital from November 2016 to March 2018 were selected as the study subjects. They were divided into young and middle-aged group and elderly groups, according to different age, 30 cases in each group. The younger and middle-aged group were colorec- tal cancer patients under 60 years old, and the older group were colorectal cancer patients over 60 years old. The clinicopathological characteristics of the two groups were analyzed. Results There was no significant difference between the young and middle-aged group and the old group in terms of operation time, operation method, sex, duration of illness, length of hospitalization and amount of bleeding during operation. There was no significant difference between the young and middle-aged group and the old group in terms of phase A. In terms of phase B, phase C and phase D, there was no significant difference between the young and middle-aged group and the old group. There was no significant difference in the long-term survival rate between the elderly group and the young and middle-aged group (P<0.05). There was significant difference in pathological morphology, depth of in- vasion of primary tumors and histological types between the young and middle-aged group and the old group (P<0.05). Conclusion In terms of clini- cal symptoms, there is no significant difference between colorectal cancer in the elderly and that in the young and middle-aged. Therefore, it is neces- sary to diagnose colorectal cancer in the elderly and middle-aged as early as possible according to the habits of colonoscopy, shape and diet of the pa- tients, so as to improve the prognosis of the patients.
