纤支镜下灌洗联合布地奈德与氨溴索治疗儿童肺炎支原体肺炎的临床观察

Clinical observation of bronchoscopic lavage combined with budesonide and ambroxol in the treatment of Mycoplasma pneumoniae pneumonia in children

目的探讨纤支镜下灌洗联合布地奈德与氨溴索治疗儿童肺炎支原体肺炎的临床效果.方法按随机数字表法将2017年1月至2019年1月就诊于本院的儿童肺炎支原体肺炎患儿90例分为对照组和治疗组,每组45例.对照组行阿奇霉素静滴联合对症治疗,治疗组在此基础上行纤支镜下灌洗联合布地奈德、氨溴索治疗.对比两组临床疗效、实验室指标、临床症状改善时间、住院时间及不良反应.结果与对照组相比,治疗组治疗总有效率较高,差异有统计学意义(P<0.05);治疗组白细胞计数(WBC)、C反应蛋白(CRP)水平低于对照组,差异有统计学意义(P<0.05);与对照组相比,治疗组肺部啰音、体温、咳嗽等改善时间和住院时间较短,差异有统计学意义(P<0.05);两组患儿均未出现严重的不良反应.结论纤支镜下灌洗联合布地奈德、氨溴索可有效改善儿童肺炎支原体肺炎患儿肺部体征,缩短临床症状改善时间,不良反应少,安全性高,值得临床推广.

Objective To investigate the clinical effect of bronchoscopic lavage combined with budesonide and ambroxol in the treatment of My- coplasma pneumoniae pneumonia in children. Methods 90 children with Mycoplasma pneumoniae pneumonia from January 2017 to January 2019 were divided into control group and treatment group, with 45 cases in each group. The control group received azithromycin intravenous drip com- bined with symptomatic treatment, while the treatment group received bronchoscopic lavage combined with budesonide and ambroxol. The clinical efficacy, laboratory indicators, improvement time of clinical symptoms, hospitalization time and adverse reactions were compared between the two groups. Results Compared with the control group, the total effective rate of the treatment group was higher, with statistical significance (P<0.05). WBC and CRP levels of the treatment group were lower than those of the control group, with statistical significance (P<0.05). Compared with the control group, the lung rale, body temperature and cough of the treatment group were improved. Good time and hospitalization time were shorter, the difference was statistically significant (P<0.05);there were no serious adverse reactions in the two groups. Conclusion Fiberoptic bronchoscopic lavage combined with budesonide and ambroxol can effectively improve the pulmonary signs of children with Mycoplasma pneumoniae pneumonia, shorten the improvement time of clinical symptoms, less adverse reactions and high safety, which is worthy of clinical promotion.
