新型中央孔型有晶状体眼后房型人工晶状体（ICL V4c）治疗高度近视的视觉质量分析

Analysis of visual quality after implantable collamer lens with a central hole(ICL V4c) for the correction of high myopia

目的探讨新型中央孔型有晶状体眼后房型人工晶状体(ICL V4c)植入术治疗高度近视患者的视觉质量变化.方法选取2018年7月至2019年3月在中国人民解放军第四七四医院接受ICL V4c植入术的高度近视患者58例(116眼)为研究对象.所有患者随访至术后6个月,分别测量治疗后视力、眼压、调制传递函数截止频率(MTF,cut off)、客观散射指数(OSI)、斯特列尔比(SR)、不同对比度下OQAS值(OV100%、OV20%、OV9%).结果术后1、3、6个月的视力恢复情况较术前逐渐提高,差异有统计学意义(P<0.05).术后1、3、6个月的MTF、OSI、SR与术前比较均提高,但差异无统计学意义.术后1、3、6个月的OV100%、OV20%、OV9%与术前比较均明显提高,差异均有统计学意义(P<0.05).所有患者术眼术后有4眼发生高眼压,经降眼压对症治疗后眼压恢复正常,余术眼术后眼压与术前比较,差异均无统计学意义.结论ICL V4c植入术能有效改善高度近视患者视力及屈光度,术后早期视觉质量在OV100%、OV20%、OV9%方面均优于术前.

Objective To analyze the visual quality changes of implantabIe collamer lens(ICL)with a central hole(ICL V4c)on high myopia pa-tients.Methods Fifty-eight patients(116 eyes)with high myopia who underwent ICL V4c implantation in our hospital from July 2018 to March 2019 were selected.All patients were followed up for 6mo after operation,after treatment,visual acuity,intraocular pressure,modulation transfer function(MTF)cut off frequency,objective scattering index(OSI),Strelby ratio(SR),and OQAS values(OV100%,OV20%,OV9%)were mea-sured.Results The visual acuity recovery at 1,3 and 6 months after treatment were better than those before treatment,and the differences were sta-tistically significant(P<0.05).MTF cut off,OSI,SR of postoperative 1,3 and 6 months were all improved compared with that of preoperative,but the difference was not statistically significant.The OV100%,OV20%and OV9%of the patients with 1,3 and 6 months after operation were signifi-cantly improved compared with that before operation,and the differences were statistically significant(P<0.05).There were 4 eyes with high intra-ocular pressure after treatment,and the intraocular pressure returned to normal after symptomatic treatment of lowering intraocular pressure.There were no significant differences between the intraocular pressure after treatment and that before treatment.Conclusion ICL V4c implantation can ef-fectively improve the visual acuity and diopter of patients with high myopia.Early visual quality after operation is better than that before operation in OV100%,OV20%,OV9%.
