耳穴贴磁对卒中后睡眠障碍患者的疗效观察 被引量:8

摘要 目的:探讨耳穴贴磁治疗脑卒中后睡眠障碍的临床效果。方法:将93例脑卒中后睡眠障碍患者随机分为常规治疗组(简称常规组)、常规+磁疗组、常规+王不留行籽组,每组31例。三组患者均接受常规治疗方案,在此基础上,常规+磁疗组、常规+王不留行籽组患者分别予以磁珠和王不留行籽耳穴贴压,耳穴选取神门、皮质下、交感、心、肾、枕、垂前,每日按压4次(三餐后及睡前),每3天更换另一侧耳郭,两耳交替,治疗4周。治疗前后采用匹兹堡睡眠质量指数(PSQI)进行评定。结果:共83例完成试验。治疗4周后,三组PSQI总分有显著性差异(P<0.01),常规+磁疗组睡眠质量的提高优于常规+王不留行籽组(P<0.05),包括入睡时间,日间功能障碍维度(P<0.05);常规+磁疗组、常规+王不留行籽组睡眠质量的提高明显优于常规治疗组(P<0.01),包括睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍维度(P<0.01或0.05)。结论:耳穴贴磁、王不留行籽耳穴贴压均可一定程度改善脑卒中后睡眠障碍患者的睡眠状况,耳穴贴磁的疗效优于王不留行籽贴压。 Objective:To explore the clinical effect of improving sleep quality through magnetic bead acupressure on auricular point for stroke patients with sleep disorder. Method:93 patients with post-stroke sleep disorder were randomly divided into a conventional group(a control group),a conventional combined with magnetotherapy group(an observation 1 group),a conventional combined with vaccaria seed group(an observation 2 group),31 cases in each group. Conventional treatments were applied in the three groups.On the basis of conventional treatments,auricular magnetotherapy was added in the observation 1 group,and in the observation 2 group,vaccaria seed was applied at shenmen,subcortex,sympathy,heart,kidney,pillow and chuiqian,pressed 4 times a day(after three meals and before bedtime),replace the other auricle every 3 days,alternate ears and treating for 4 weeks. Before and after the test,the scale of PSQI was adopted to assess the sleep quality of all participants. Result: A total of 83 cases completed the test.After 4 weeks of treatment,there were significant differences in the total scores of PSQI between the three groups(P<0.01).The improvement of sleep quality in the observation 2 group was better than that in the observation 1 group(P<0.05),including sleep latency and daytime dysfunction dimension(P<0.05). The improvement of sleep quality in the observation 2 group and the observation 1 group was significantly better than that in the control group(P<0.01),including sleep quality,sleep latency,sleep time,sleep efficiency,sleep disorder,and daytime dysfunction dimension(P<0.01 or 0.05). Conclusion: Auricular magnetotherapy and vaccaria seed pressing could improve the sleep quality of post-stroke patients. The auricular magnetotherapy effect was superior to the vaccaria seed pressing.
