摘要
为了揭示呼吸道合胞病毒(respiratory syncytial virus,RSV)感染患儿的血清细胞因子水平与体重、分娩方式和喘息的相关性。本研究选取了我院收治的417例下呼吸道感染患儿进行研究,其中包括132例RSV感染患儿(RSV组)和285例非RSV感染患儿(non-RSV组)。通过流式细胞术检测受试婴儿血清IFN-γ、TNF-α、IL-2、IL-4、IL-6和IL-10水平。研究显示,下呼吸道感染患儿的TNF-α、IFN-γ、IL-2、IL-4、IL-6和IL-10水平均显著高于健康婴儿。RSV组患儿的血清TNF-α和IL-6水平显著高于非RSV组患儿(p<0.05)。与出生体重≥3.0 kg(正常出生体重)的RSV感染患儿相比,出生体重<3.0 kg(低出生体重)的RSV感染患儿的TNF-α和IL-6水平显著升高(p<0.05)。另外,与未出现喘息症状的RSV感染患儿相比,伴有喘息症状的RSV感染患儿的TNF-α和IL-6水平显著升高(p<0.05)。在伴有喘息的RSV感染婴儿中,低出生体重的患儿的TNF-α和IL-6水平显著升高,均为所有亚组中最高值(p<0.05)。然而,分娩方式对患儿的血清细胞因子水平无显著影响(p>0.05)。本研究表明,TNF-α和IL-6涉及RSV感染患儿的喘息或哮喘的发病机制,尤其是低出生体重患儿。
To rev eal the relationship between serum cytokine levels and body weight,mode of delivery and wheezing in infants with respiratory syncytial virus(RSV)infection,this study selected 417 infants with lower respiratory tract infections in our hospital,including 132 infants with RSV infection(RSV group)and 285 infants with non-RSV infection(non-RSV group).Serum levels of IFN-γ,TNF-α,IL-2,IL-4,IL-6 and IL-10 were measured by flow cytometry.Studies have shown that infants with lower respiratory tract infections have significantly higher levels of TNF-α,IFN-γ,IL-2,IL-4,IL-6,and IL-10 than healthy infants.Serum TNF-α and IL-6 levels were significantly higher in the RSV group than in the non-RSV group(p<0.05).The levels of TNF-α and IL-6 in RSV-infected infants with birth weight<3.0 kg(low birth weight)were significantly higher than those in RSV-infected infants with birth weight<3.0 kg(normal birth weight)(p<0.05).In addition,TNF-α and IL-6 levels were significantly elevated in RSV-infected infants with wheezing symptoms compared with RSV-infected infants without wheezing symptoms(p<0.05).In RSV-infected infants with wheezing,the levels of TNF-αand IL-6 in low birth weight infants were significantly higher than those in all subgroups(p<0.05).However,the mode of delivery had no significant effect on serum cytokine levels(p>0.05).This study suggests that TNF-α and IL-6 are involved in the pathogenesis of wheezing or asthma in infants with RSV infection,especially in infants with low birth weight.
