局部皮瓣移植在小儿耳前瘘管切除术后创面修复中的应用

The Application of Local Skin Flap Transplantation in the Repair of Wounds after Anterior Auricular Fistulectomy in Children

目的:探究局部皮瓣移植应用于小儿耳前瘘管切除术后创面修复的治疗效果。方法:选取2016年2月-2019年2月于笔者医院小儿耳鼻喉科就诊的70例小儿耳前瘘管患儿,按照随机数字表随机分为观察组和对照组。观察组35例,小儿耳前瘘管切除术后创面应用移植皮瓣修复;对照组35例,小儿耳前瘘管切除术后常规缝合治疗。对比两组治疗后的恢复时间、患儿心理状态、生活质量评分和不良反应发生情况。结果:治疗后,观察组脓肿愈合时间、住院时间、SAS评分、SDS评分和复发率均低于对照组,差异有统计学意义(P<0.05);观察组躯体功能、心理功能、社会功能、物质生活评分均高于对照组,差异有统计学意义(P<0.05)。结论:手术切除后采用局部皮瓣移植修复相对于单一小儿耳前瘘管切除术,术后修复效果更好,减少了术后脓肿发生率,提高了患儿生活质量,减少了术后复发率。

Objective To explore the therapeutic effect of local skin flap transplantation in children after anterior auricular fistula resection.Methods Seventy children with preauricular fistula between February 2016 and February 2019,according to random number table,they were randomly divided into the observation group and the control group.In the observation group,35 cases were repaired with skin flap after anterior auricular fistula resection.In the control group,35 cases were treated with routine suture after resection of preauricular fistula.The recovery time,psychological state,quality of life score and adverse reactions were compared between the two groups.Results After treatment,the abscess healing and hospitalization time,SAS score,SDS score and recurrence rate in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The scores of physical function,psychological function,social function and material life in the observation group were higher than those in the control group(P<0.05).Conclusion Compared with the single removal of preauricular fistula in children,the local skin flap transplantation after the operation has a better effect,reduces the incidence of postoperative abscess,improves the quality of life of children,and reduces the recurrence rate.
