重组人干扰素a-2b注射液及其凝胶治疗外阴并宫颈HPV亚临床感染的效果

Effect of Recombinant Human Interferon α-2b Injection and Gel on Subclinical Vulvar and Cervical HPV Infection

目的探讨重组人干扰素a-2b肌内注射及其凝胶外用对外阴并宫颈感染人乳头瘤病毒(HPV)患者的效果。方法选取84例外阴及宫颈HPV亚临床感染的患者,按随机数字表法将其分为对照组和治疗组,每组42例。对照组外阴及宫颈予重组人干扰素a-2b凝胶外用;治疗组在对照组基础上,同时予重组人干扰素a-2b肌内注射,治疗1个月为一疗程,共1~2个疗程。采用PCR+膜杂交法进行HPV分型(21型)检测,分析HPV各分型感染的发生频率,并比较2组治疗1、2个月时HPV转阴率及不良反应。结果84例患者共检测出21种HPV型别,其中50例患者外阴与宫颈感染的HPV亚型一致。外阴和宫颈高危亚型HPV感染分别占69.77%、76.53%,其中高危亚型HPV16感染居首位,外阴和宫颈感染分别占13.38%、11.73%;其次为低危亚型HPV6感染,外阴和宫颈感染分别占12.79%、10.20%。治疗组HPV转阴率高于对照组(治疗1个月:88.10%比66.67%,P<0.05;治疗2个月:97.61%比83.33%,P<0.05)。治疗组13例患者出现一过性发热、寒战、头痛、全身乏力及肌肉酸痛,未影响治疗。结论重组人干扰素a-2b肌内注射及其凝胶外用治疗外阴并宫颈HPV亚临床感染疗效确切,不良反应小。

Objective To investigate the effect of intramuscular recombinant human interferonα-2b injection combined with its gel formulation on vulvar and cervical human papillomavirus(HPV)infection.Methods Eighty-four patients with subclinical vulvar and cervical HPV infection were randomly given 1-21-month courses of treatment with recombinant human interferonα-2b by gel for external use alone(control group,n=42)or in combination with intramuscular injection(treatment group,n=42).HPV types(21 types)were detected by PCR+membrane hybridization method to analyze the frequency of HPV infection in each type.In addition,the negative conversion rate and adverse reactions were observed after 1 and 2 months of treatment.Results A total of 21 HPV types were detected in the 84 patients.HPV subtypes of vulvar infection were consistent with those of cervical infection in 50 patients.The incidences of high-risk subtypes of vulvar and cervical HPV infection were 69.77%and 76.53%,respectively.The predominant high-risk type was HPV-16(13.38%for vulvar infection and 11.73%for cervical infection),followed by HPV6(12.79%for vulvar infection and 10.20%for cervical infection).The negative conversion rate of HPV in treatment group was higher than that in control group(88.10%vs 66.67%after 1 month of treatment;97.61%vs 83.33%after 2 months of treatment,P<0.05).Thirteen patients in treatment group presented with transient fever,chills,headache,systemic fatigue and muscle pain,which did not affect the treatment.Conclusion Intramuscular recombinant human interferonα-2b injection combined with its gel formulation is an effective treatment with minimal adverse reactions for subclinical vulvar and cervical HPV infection.
