妊娠晚期子痫前期发生的影响因素及其严重程度与凝血功能指标、血小板参数的相关性分析

Analysis of influencing factors of preeclampsia and correlation between severity of preeclampsia and coagulation indexes and platelet parameters in late pregnancy

目的分析妊娠晚期子痫前期发生的影响因素及其严重程度与凝血功能指标、血小板参数的相关性。方法选择50例妊娠晚期子痫前期孕妇作为观察组,另选同期产检的50例健康孕妇作为对照组。分析妊娠晚期子痫前期发生的影响因素,妊娠晚期子痫前期疾病严重程度与凝血功能指标、血小板参数的相关性。结果两组孕妇年龄、受教育程度、孕周比较,差异无统计学意义(P>0.05);两组孕妇高血压病史、体质量指数、凝血酶原时间、活化部分凝血活酶时间、纤维蛋白原、血小板计数、血小板平均体积、血小板分布宽度比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,有高血压病史、体质量指数高为妊娠晚期子痫前期发生的独立危险因素(P<0.05)。妊娠晚期子痫前期疾病严重程度与血小板计数、凝血酶原时间、活化部分凝血活酶时间呈负相关,与血小板平均体积、血小板分布宽度、纤维蛋白原呈正相关(r=-0.67、-0.42、-0.55、0.59、0.58、0.47,P<0.05)。结论有高血压病史、体质量指数高为妊娠晚期子痫前期发生的危险因素,凝血功能指标、血小板参数与妊娠晚期子痫前期的疾病严重程度密切相关。

Objective To analyze the influencing factors of preeclampsia and correlation between severity of preeclampsia and coagulation indexes and platelet parameters in late pregnancy.Methods There were 50 pregnant women Mdth preeclampsia in late pregnancy selected as the observation group,and 50 healthy pregnant women in the same period selected as the control group.The influencing factors of preeclampsia in late pregnancy were analyzed,and the correlation between the severity of preeclampsia in late pregnancy and coagulation indexes and platelet parameters were analyzed.Results There was no statistically significant difference in age,education level,gestational age between the two groups(P>0.05).There was statistically significant difference in history of hypertension,body mass index,prothrombin time,activated partial thromboplastin time,fibrinogen,platelet count,mean platelet volume,platelet distribution width between the two groups(P<0.05).Multivariate Logistic regression analysis showed that history of hypertension and high body mass index were independent risk factors of preeclampsia in late pregnancy(P<0.05).The severity of preeclampsia in late pregnancy was negatively correlated Math platelet count,prothrombin time and activated partial thromboplastin time,and positively correlated with mean platelet volume,platelet distribution width and fibrinogen(r=-0.67,-0.42,-0.55,0.59,0.58,0.47,P<0.05).Conclusion History of hypertension and high body mass index are risk factors of preeclampsia in late pregnancy.Coagulation indexes and platelet parameters are closely correlated with the severity of preeclampsia in late pregnancy.
