广东省居民职业体检中恶性肿瘤重点筛查人群环境暴露风险及相关因素分析

Risk of environmental exposure and related factors of malignant tumor screening population in occupational physical examination of Guangdong residents

目的分析广东省居民肿瘤发病的相关因素并评价职业暴露的风险。方法采用横断面的调查研究方法,在2018年7月—9月间对2018年广东药科大学附属第一医院在广州、肇庆、梅州、汕头进行职业体检的广东省内居民进行肿瘤发病相关症状和职业暴露的面访式调查。采用Logistics回归分析进行自我报告职业暴露程度和肿瘤重点筛查发生率的相关性。结果本次共调查5861名职业人群,其中男占85.8%;女占14.2%;职业环境暴露种类(程度)(OR=1.374,P<0.001)、工作压力(OR=2.268,P<0.001)、是否饮食规律、年龄及恶性肿瘤家族史均与肿瘤重点筛查人群相关。结论肿瘤重点筛查人群分类重要的依据是环境暴露种类数、工作压力及肿瘤家族史,应限制同一个人在同一个时期遭受3种以上的职业环境暴露,关注肿瘤家族史及高工作压力人群。

Objective To analyze the related factors of cancer incidence in Guangdong residents and evaluate the risk of occupational exposure.Methods A cross-sectional survey was conducted to investigate the symptoms and occupational exposure of cancer patients in Guangzhou,Zhaoqing,Meizhou and Shantou in the First Affiliated Hospital of Guangdong Pharmaceutical University from July to September 2018.Logistic regression analysis was used to analyze the correlation between self-reported occupational exposure and the incidence of major cancer screening.Results A total of 5861 occupational groups were investigated,of which 85.8%were males and 14.2%were females.The types and degrees of occupational environmental exposure(OR=1.374,P<0.001),working pressure(OR=2.268,P<0.001),dietary pattern,age and family history of malignant tumors were all related to the key cancer screening population.Conclusion The important basis for classification of cancer screening population is the type of environmental exposure,work stress and family history of cancer.The same person should be limited to more than three kinds of occupational environmental exposure at the same time,and attention should be paid to family history of cancer and high work stress population.
