皮肤恶性黑色素瘤组织中MMP-9和Tiam1和Nm23-h1和Caveolin-1的表达及临床意义

Expression and clinical significance of MMP-9,Tiam1,Nm23-h1 and Caveolin-1 in patients with cutaneous malignant melanoma

目的探讨皮肤恶性黑色素瘤患者组织中MMP-9、Tiam1、Nm23-h1和Caveolin-1的表达和临床意义。方法选取2010年1月至2018年12月间宝鸡市联勤保障部队第九八七医院收治的45例皮肤恶性黑色素瘤患者的肿瘤存档蜡块进行研究,另选取同期皮肤交界痣20例及皮肤性切除的正常皮肤组织20例为对照,对3组组织中的MMP-9、Tiam1、Nm23-h1和Caveolin-1表达进行检测,分析各指标水平与临床病理特征的关系。结果恶性黑色素瘤皮肤的MMP-9和Tiam1阳性表达高于交界痣及正常皮肤,Nm23-h1和Caveolin-1阳性表达低于交界痣及正常皮肤;交界痣的Tiam1阳性表达高于正常皮肤,Nm23-h1阳性表达低于正常皮肤,差异均有统计学意义(均P<0.05),交界痣与正常皮肤的MMP-9和Caveolin-1阳性表达比较,差异无统计学意义(P>0.05)。随着皮肤恶性黑色素瘤浸润深度的增加,MMP-9和Tiam1阳性表达逐渐增加,Nm23-h1和Caveolin-1阳性表达逐渐下降;伴淋巴结转移患者的MMP-9和Tiam1阳性表达高于不伴淋巴结转移者,Nm23-h1和Caveolin-1阳性表达低于不伴淋巴结转移者,差异均有统计学意义(均P<0.05)。结论MMP-9、Tiam1、Nm23-h1和Caveolin-1的表达水平在皮肤恶性黑色素瘤的发生、发展中起重要作用。

Objective To investigate the expression and clinical significance of MMP-9,Tiam1,Nm23-h1 and Caveolin-1 in patients with cutaneous malignant melanoma.Methods Wax blocks for malignant melanoma were collected from forty-five patients with cutaneous malignant melanoma treated at The987 th Hospital of Baoji Joint Logistic Support Force from January 2010 to December 2018,and another twenty patients with melanocytic nevus and twenty patients with normal skin tissues were selected at the same time as the control group.The expressions of MMP-9,Tiam1,Nm23-h1 and Caveolin-1 were detected among the three groups.The relationship between the levels of indexes and their clinicopathological characteristics were analyzed.Results The positive expression of MMP-9 and Tiam1 was significantly higher in patients with cutaneous malignant melanoma than patients with melanocytic nevus and patients with normal skin,and the expression of Nm23-h1 and Caveolin-1 was were significantly lower in patients with cutaneous malignant melanoma than patients with melanocytic nevus and patients with normal skin(all P<0.05).The positive expression of Tiam1 was significantly higher in patients with melanocytic nevus than in patients with normal skin tissues while the positive expression of Tiam1 was significantly lower in patients with melanocytic nevus than in patients with normal skin tissues(all P<0.05).There was no significant difference in the expression of MMP-9 and Tiam1 between patients with melanocytic nevus and patients with normal skin tissues(P>0.05).With the increased infiltration depth of malignant melanoma,the positive expression of MMP-9 and Tiam1 increased gradually,and the positive expressions of Nm23-h1 and Caveolin-1 decreased gradually.The positive expressions of MMP-9 and Tiam1 in patients with lymph node metastasis were significantly higher than those without lymph node metastasis,and the positive expressions of Nm23-h1 and Caveolin-1 were significantly lower in patients with lymph node metastasis than in patients without lymph node metastasis(all P<0.05).Conclusion The expression levels of MMP-9,Tiam1,Nm23-h1 and Caveolin-1 plays an important role in the occurrence and development of cutaneous malignant melanoma.
