彩色多普勒超声对胆管癌诊断的价值分析

Significance of color Doppler ultrasonography in the diagnosis of bile duct carcinoma

目的探讨彩色多普勒超声对胆管癌的诊断及鉴别诊断价值。方法选取2016年1月至2019年1月间西安市第五医院收治的100例胆管癌患者,均进行彩色多普勒超声及病理学检查,对检查结果进行比较分析。结果患者经彩色多普勒超声诊断,肝内病变38例,肝外病变62例。38例肝内病变患者中,胆管细胞癌22例,肝细胞癌10例,胆管结石合并胆管炎4例,肝脓肿2例,肝内胆管癌超声诊断准确率为57.9%(22/38);62例肝外病变患者中,肝外胆管癌54例,胰腺癌4例,胆固醇结石2例,漏诊2例,肝外胆管癌超声诊断准确率为87.1%(54/62)。结论彩色多普勒超声对胆管癌进行诊断及鉴别诊断中,均有重要的临床诊断价值,值得推广.

Objective To study the value of color Doppler ultrasonography in the diagnosis of bile duct carcinoma.Method A total of 100 patients with bile duct carcinoma treated at The Fifth Hospital of Xi’an during the period from January 2016 to January 2019 were selected as the research subjects.Color Doppler ultrasonography examination and pathological examination were performed and the examination results were compared between the two groups.Results According to Color Doppler ultrasound,38 patients were found to have intrahepatic lesions and 62 patients were found to have extrahepatic lesions.A total of 22 patients were found to have cholangiocarcinoma,10 patients were found to have hepatocellular carcinoma,4 patients were found to have choledocholithiasis and cholangitis and 2 patients were found to have liver abscess among the 38 patients with intrahepatic lesions.The accuracy rate of ultrasound for the diagnosis of intrahepatic bile duct carcinoma was 57.9%(22/38).Fifty-four patients were found to have extrahepatic bile duct carcinoma,4 patients were found to have pancreatic cancer,2 patients were found to have cholesterol gallstones and 2 patients missed the diagnosis among the 62 patients with extrahepatic lesions.The accuracy rate of ultrasound for the diagnosis of extrahepatic bile duct carcinoma was 87.1%(54/62).Conclusion Color Doppler ultrasound has the important clinial diagnosis significance in the diagnosis of bile duct carcinoma,and is worthy of clinical promotion.
