基于因子分析的老年社区获得性肺炎中医证候特点

Characteristics of TCM Syndrome of Aged Community Acquired Pneumonia Based on Factor Analysis

目的采用因子分析方法探讨老年社区获得性肺炎的中医证候规律,为该病的辨证论治提供依据。方法采用前瞻性调查研究的方法,收集天津市东丽医院、天津中医药大学第二附属医院204例老年社区获得性肺炎患者,运用因子分析法对其证候信息进行“降维”。结果通过因子分析的方法,得出6个公因子,命名如下:F1:痰浊阻滞兼肺脾气虚证;F2:痰阻气滞证;F3:热盛伤津证;F4:痰热郁肺兼气阴两虚证;F5:肺胃炽热证;F6:脾气亏虚证。结论老年社区获得性肺炎证候以痰热郁肺为主,同时兼夹有肺、脾、胃等脏腑气虚、阴虚的特点。

Objective:To explore the TCM syndrome pattern of community-acquired pneumonia in the elderly by factor analysis,and to provide evidence for syndrome differentiation and treatment of the disease.Methods:A prospective investigation and research method was used to collect 204 elderly patients with community-acquired pneumonia from Dongli Hospital of Tianjin City and The Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine.The factor analysis method was used to reduce the dimension of their syndrome information.Results:6 factor factors were obtained by the factor analysis and named as follows:F1:phlegm turbidity block concurrent with lung-spleen qi deficiency syndrome;F2:phlegm block qi stagnation syndrome;F3:fluid consumption syndrome due to intense heat;F4:phlegm-heat lung stagnation concurrent with phlegm-heat stagnating lung syndrome and qi and yin deficiency syndrome;F5:lung-stomach fever syndrome;F6:spleen qi deficiency syndrome.Conclusion:The syndrome of acquired pneumonia in the elderly community is mainly characterized by phlegm-heat by stagnating lung and it also has the characteristics of qi and yin deficiency in lung,spleen and stomach.
