摘要
目的评估颈动脉彩色多普勒超声检查对缺血性脑卒中的临床价值。方法224例缺血性脑卒中患者纳入研究,同期144例健康体检者作为对照组。受试者均接受彩色多普勒超声检查,观察2组受试者颈动脉内膜-中膜厚度(IMT),斑块以及血流动力学情况。结果研究组患者斑块、狭窄和闭塞情况的发生率均显著高于对照组(χ^2=123.97,50.95,12.17;P<0.05)。研究组颈动脉IMT[(1.09±0.18)mm],显著高于对照组[(0.62±0.12)mm,t=27.63,P<0.05]。研究组IMT增厚发生率,双侧斑块和多发斑块发生率,脂质性软斑块和混合型斑块发生率显著高于对照组(P<0.05)。研究组搏动指数(PI)和阻力指数(RI)均显著高于对照组(P<0.05)。结论彩色多普勒超声检查有助于了解缺血性脑卒中患者的病情程度、提供诊断和治疗依据、判断病程及预后,具有较高的临床价值。
Objective To investigate the clinical value of carotid color Doppler ultrasonography in evaluating the severity of ischemic cerebral stroke.Methods A total of 224 patients with ischemic cerebral stroke were enrolled in the study,at the same time,the other 144 healthy subjects were enrolled as control group.All the subjects were examined by color Doppler ultrasonography.The carotid intima-media thickness(IMT),plaque and hemodynamics were observed and compared between the two groups.Results The incidence rates of plaque,stenosis and occlusion in research group were significantly higher than those in control group(P<0.05).The IMT of carotid artery in research group was(1.09±0.18)mm,which was significantly higher than that(0.62±0.12)mm in control group(P<0.05).The incidence rates of IMT thickening,bilateral and multiple plaques as well as lipid soft plaque and mixed plaque in research group were significantly higher than those in control group(P<0.05).Moreover the pulsation index(PI)and resistance index(RI)in research group were significantly higher than those in control group(P<0.05).Conclusion Color Doppler ultrasonography is helpful to understand the severity of ischemic cerebral stroke,provide the basis for diagnosis and treatment,judging the course and prognosis,therefor,which has higher clinical application value.
