呼吸道病原体谱抗体IgM检测在呼吸道感染诊断中应用价值 被引量:10

摘要 目的分析呼吸道感染患者常见的11种呼吸道病原体抗体IgM检出情况。方法选取2018年1-12月北部战区总医院收治的3 128例呼吸道感染患者为研究对象。采用间接免疫荧光法检测呼吸道合胞病毒(RSV)、腺病毒(ADV)、流感病毒B(INFB)、流感病毒A(INFA)﹑副流感病毒(PIV)﹑噬肺军团菌(LP)﹑肺炎衣原体(CP)﹑肺炎支原体(MP)﹑柯萨奇病毒A(CoxA)、柯萨奇病毒B(CoxB)﹑埃可病毒(ECHO)特异性IgM。比较不同性别、不同季节、不同年龄患者IgM阳性检出率。结果病原体谱抗体IgM阳性率为58.2%(1 822/3 128)。被检患者血清中INFB最多,阳性率为35.9%(1 124/3 128);其次为CP,阳性率为21.5%(672/3 128);LP最少,阳性率为0.5%(15/3 128)。男性组阳性检出率为56.7%(1 055/1 861),低于女性组的60.5%(767/1 267),差异有统计学意义(P<0.05)。春季阳性率最高,达68.9%(602/874),其次是冬季,阳性率为61.2%(540/882)。夏季、秋季阳性率分别为53.2%(362/680)、46.0%(318/692)。各季节阳性检出率比较,差异均有统计学意义(P<0.05)。各年龄组阳性检出率比较,差异有统计学意义(P<0.05)。结论通过11种呼吸道病原体IgM检测能正确区分不同早期呼吸道病原体感染,为临床及早预防和诊治提供有力依据。 Objective To analyze IgM detection of 11 common respiratory pathogen antibodies in patients with respiratory tract infection.Methods A retrospective study was performed on 3 128 cases of patients with respiratory tract infection who were admitted from January to December 2018.Indirect immunofluorescence assay was used to detect the specific IgM of respiratory syncytial virus(RSV),adenovirus(ADV),influenza virus B(INFB),influenza virus A(INFA),para-influenza virus(PIV),legionella pneumophila(LP),chlamydia pneumoniae(CP),mycoplasma pneumoniae(MP),coxazi virus A(CoxA),coxazi virus B(CoxB),Echovirus(ECHO).The positive detection rate of patients with different gender,season and age were compared.Results The positive rate of pathogen antibody IgM was 58.2%(1 822/3 128).The most INFB was detected in the serum of the tested patients,with a positive rate of 35.9%(1 124/3 128).CP was the second,with a positive rate of 21.5%(672/3 128).LP was the least,with a positive rate of 0.5%(15/3 128).The positive detection rate of the male group was 56.7%(1 055/1 861),lower than that of the female group(60.5%,767/1 267),and the difference was statistically significant(P<0.05).The highest positive rate was 68.9%(602/874)in spring,followed by 61.2%(540/882)in winter.The positive rates in summer and autumn were 53.2%(362/680)and 46.0%(318/692),respectively.The positive detection rate of each season was statistically significant(P<0.05).The positive detection rate of each age group was statistically significant(P<0.05).Conclusion IgM detection of 11 respiratory pathogens can correctly distinguish different early respiratory pathogen infections,providing a strong basis for early clinical prevention and treatment.
