摘要
目的探讨种植体周围龈沟液(PICF)中miR-23a、miR-146a水平与种植体周围炎的关系。方法选取行牙种植修复,并进行术后12个月复查的189例患者为研究对象,根据临床指标将患者分为:健康种植体组87例、炎症种植体组102例,同期选取参与体检者78例作为对照组,检测并记录牙种植修复患者及正常人探针深度(PD)、龈沟出血指数(SBI)、龈沟液量(GCF)的值,采用qRT-PCR法检测龈沟液中miR-23a、miR-146a水平,采用Pearson相关性分析检测龈沟液中miR-23a、miR-146a水平与各临床指标的关系。结果炎症种植体组PD、GCF、SBI均显著高于对照组和健康种植体组(P<0.05);炎症种植体组患者龈沟液中miR-23a、miR-146a水平显著高于对照组和健康种植体组(P<0.05),健康种植体组患者龈沟液中miR-23a、miR-146a水平显著高于对照组(P<0.05);不同性别患者种植体周围龈沟液中miR-23a、miR-146a水平差异无统计学意义(P>0.05);>60岁组患者种植体周围龈沟液中miR-23a、miR-146a水平显著高于<40岁组和40~60岁组;龈沟液中miR-23a、miR-146a水平均与患者PD、SBI呈正相关(P<0.05)。结论种植体周围龈沟液中miR-23a、miR-146a水平与种植体周围炎患者临床指标有关,可能成为反映种植体周围炎症进展及牙周组织损伤程度的生物标志物。
Objective To investigate the correlation between the levels of miR-23 a and miR-146 a in peri-implant gingival crevicular fluid(PICF)with peri-implant inflammation.Methods All 189 patients who received dental implant prosthesis were enrolled and reviewed at 12 months after operation.According to the clinical indicators,the patients were divided into 87 cases in healthy implant group and 102 cases in inflammatory implant group;in the same period,78 patients who participated in physical examination in our hospital were selected as the control group.The values of probe depth(PD),sulcus bleeding index(SBI)and gingival crevicular fluid volume(GCF)were measured and recorded in patients with dental implant prosthesis and normal subjects.The levels of miR-23 a and miR-146 a in gingival crevicular fluid were detected by qRT-PCR.Pearson correlation analysis was used to detect the correlations between the levels of miR-23 a and miR-146 a in gingival crevicular fluid with clinical indicators.Results PD,GCF and SBI of inflammatory implant group were significantly higher than those of control group and healthy implant group(P<0.05).The levels of miR-23 a and miR-146 a in gingival crevicular fluid of patients in inflammatory implant group were significantly higher than those in control group and healthy implant group(P<0.05).The levels of miR-23 a and miR-146 a in gingival crevicular fluid of healthy implant group were significantly higher than those of control group(P<0.05).There was no significant difference in the levels of miR-23 a and miR-146 a in peri-implant gingival crevicular fluid between male and female patients(P>0.05).The levels of miR-23 a and miR-146 a in peri-implant gingival crevicular fluid in patients older than 60 years were significantly higher than those in patients aged younger than 40 years and between 40-60 years.The levels of miR-23 a and miR-146 a in gingival crevicular fluid were positively correlated with PD and SBI(P<0.05).Conclusion The levels of miR-23 a and miR-146 a in peri-implant gingival crevicular fluid are correlated to the clinical indicators of peri-implant inflammation patients,which may be biomarkers reflecting the progress of peri-implant inflammation and the degree of periodontal tissue damage.
