PICU转出患儿家属迁移应激的现状及干预研究

Relocation stress and its intervention study of the families of children transferred from PICU

目的了解儿科重症监护室(pediatric intensive care unit,PICU)转出患儿家属的迁移应激现状,探索干预对策。方法采取方便抽样法,选取2018年8月PICU转出的46例患儿家属作为对照组,按常规转科流程进行健康指导,将2018年10月PICU转出的42例患儿家属作为观察组,观察组在转出前1~2 d,依托多学科团队,实施多学科延续护理。应用一般资料调查问卷和中文版ICU转出患者家属迁移应激量表分别进行问卷调查,比较迁移应激总分及满意度。结果对照组与观察组迁移应激总分分别为(51.65±6.05)分、(60.38±8.03)分;满意度分别为82.60%、92.85%,干预后PICU转出患儿家属的迁移应激水平明显降低,满意度明显提升,差异有统计学意义(P<0.05)。结论PICU转出患儿家属迁移应激处于中等程度,由多学科团队实施延续护理干预可有效降低其迁移应激水平。

Objective To understand the current status of relocation stress of the families of children transferred from PICU and explore intervention strategies.Methods Taking the convenient sampling method,the families of 46 children transferred from the PICU in August 2018 were selected as the control group.The health instruction was carried out according to the routine transfer procedure.The families of 42 children transferred from the PICU in October 2018 were selected as observation group.The observation group relied on multidisciplinary teams to implement multidisciplinary continuous care 1-2 days before the transfer.The general data questionnaire and the Chinese version of the ICU transferred out the families relocation stress scale were used to conduct a questionnaire survey on the families of the two groups,and the total relocation stress score and the satisfaction of the families were compared.Results The total relocation stress scores of the control group and the observation group were 51.65±6.05 and 60.38±8.03,respectively;and the satisfaction rates were 82.60%and 92.85%,respectively.There were significant differences in the total relocation stress scores and the satisfaction of the families between the 2 groups(P<0.05).Conclusion The relocation stress level of the families of children transferred from PICU is moderate,and continuous nursing intervention by multidisciplinary team can effectively reduce it.
