儿童慢性鼻窦炎的细菌学分布与血清学指标的相关性分析 被引量:5

摘要 目的:探讨儿童慢性鼻窦炎(CRS)的细菌学分布与血清学指标的相关性。方法:选择2015年8月至2018年2月在十堰市太和医院门诊及住院的CRS患儿78例(CRS组)及健康儿童80例(对照组),采集两组儿童的鼻腔分泌物进行细菌分离培养,检测两组患儿血清IgE表达水平并进行相关性分析。结果:CRS组患儿鼻窦分泌物培养的细菌阳性率为44. 9%,对照组为4. 6%,CRS组显著高于对照组(P<0. 05)。CRS组患儿中共分离出细菌50株,其中表皮葡萄球菌16株,产气肠杆菌10株,巴氏葡萄球菌6株,腐生葡萄球菌5株,浅绿气球菌5株,其他菌株8株。CRS组患儿的血清IgE阳性率为71. 8%,对照组为1. 3%,CRS组显著高于对照组(P<0. 05)。Spearman相关分析显示,CRS组患儿鼻窦分泌物细菌培养阳性与血清IgE阳性呈正相关(P<0. 05),在对照组中无相关性(P>0. 05)。结论:儿童CRS的鼻窦分泌物细菌培养阳性率较高,血清IgE呈现高表达状态,两者具有显著相关性,共同参与CRS的发病。 Objective: To explore the correlation between bacteriological distribution and serological markers in children with chronic rhinusinusitis( CRS). Methods: From Aug. 2015 to Feb. 2018,totally 78 children with CRS( CRS group) and 80 healthy children( control group) admitted into outpatient and inpatient departments of Taihe Children’s Hospital were selected. The nasal secretion of two groups of children were isolated and cultured,and the serum level of IgE in two groups was detected and further given the correlation analysis. Results: The positive rate of bacterial culture in nasal secretion of the CRS group was 44. 9%,while that of the control group was 4. 6%,the CRS group was significantly higher than that of the control group( P<0. 05). In the CRS group,there were 50 strains of bacteria were isolated,including 16 strains of Staphylococcus epidermidis,10 strains of Enterobacter aerogenes,6 strains of Staphylococcus pasteuriae,5 strains of Staphylococcus saprophytic,5 strains of Aerococus viridans and 8 other strains. The positive rate of serum IgE in the CRS group were 71. 8%,which were 1. 3% in the control group,and the CRS group was significantly higher than the control group( P< 0. 05). Spearman correlation analysis showed that positive bacterial culture of nasal secretions in the CRS group was positively correlated with positive serum IgE( P<0. 05),and without any positive correlation in the control group( P> 0. 05). Conclusion: The positive rate of bacterial culture of sinus secretion in children with CRS is relatively high,and the serum IgE is highly expressed. The two have significant correlation and participate in the pathogenesis of CRS.
