摘要 目的:提出基于公平性最优的大型医用设备配置规划方案,为决策者提供理论依据和参考。方法:采用优化方法中的遗传算法,考量人口和地理两方面公平性,以人口公平性为优化目标,以我国东部、中部及西部地区(31个省区市)每百万人口设备拥有量差距为约束条件,截至2018年已配置大型医用设备数量,计算出2018-2020年人口和地理公平性最优的大型医用设备配置规划方案。结果:乙类大型医用设备配置中的X射线计算机断层扫描(CT)和核磁共振(MR),运用该方法计算得出,现有规划方案中,CT和MR基于人口公平性的基尼系数分别为0.16和0.13;优化方案中,CT和MR基于人口公平性的基尼系数分别为0.12和0.10,表明优化方案的人口公平性较现有方案有所提高。结论:基于公平性最优的大型医用设备配置规划方案可为决策者进行大型医用设备配置规划提供理论依据和参考。 Objective:To propose optimal planning scheme about configuration of large medical equipments based on the fairness so as to provide theoretical basis and reference for decision maker.Methods:The genetic algorithm of optimization method was adopted and the fairness of population and geography was considered.The fairness of population was used as optimal object,and the gap of possessing amount of equipments per million population among eastern region,central region and western region(31 provinces/regions/cities)was used as constraint condition.Depended on configured amount of large medical equipments up to 2018 year to calculate the optimal planning scheme of the configuration of large medical equipments on the basis of population and geography from 2018 year to 2020 year.Results:Used CTs and MRs in the configuration of B kind of large medical equipment as examples to demonstrate the applicability of the proposed method.The calculating results of operating this method showed that the Gini coefficients of CT and MR,that were on the basis of population fairness,were 0.16 and 0.13 respectively in current planning scheme.In optimal scheme,those were 0.12 and 0.10 respectively.These indicated that the population fairness of optimal scheme was higher than that of current scheme.Conclusion:The optimal planning scheme of the configuration of large medical equipment on the basis of fairness can provide theoretical basis and reference for decision maker to make the configuration planning of large medical equipments.
