摘要
目的探讨组织切除系统(美奥舒)与传统宫腔镜诊刮对子宫内膜息肉不孕症患者冻融胚胎移植结局的影响。方法回顾性分析该院冻胚移植周期中有子宫内膜息肉手术史的76例不孕症患者的临床资料,依据子宫内膜息肉不同手术方式分为美奥舒处理组(31例)、传统宫腔镜诊刮组(45例)。比较两组患者冻胚移植后的着床率、临床妊娠率、流产率、异位妊娠率等指标。结果两组患者年龄、不孕年限、体质指数(BMI)、移植日内膜厚度、移植胚胎情况比较,差异均无统计学意义(均P>0.05);美奥舒处理组的着床率、临床妊娠率高于传统宫腔镜诊刮组,两组差异有统计学意义(P<0.05);美奥舒处理组的流产率、异位妊娠率低于传统宫腔镜诊刮组,但两组差异无统计学意义(P>0.05)。结论对于子宫内膜息肉不孕症患者,宫腔镜下组织切除系统切除术比传统宫腔镜诊刮术后的着床率及临床妊娠率更高,值得临床推广应用。
Objective To investigate the effects of two different operating methods(MyoSure hysteroscopic tissue resection and Traditional hysteroscopy and curettage)of endometrial polyps in infertility patients on outcomes of frozen-thawed embryo transfer.Methods The clinical data of 76 patients were retrospectively analyzed,who were divided into two groups according to operating methods of endometrial polyps:MyoSure group and curettage group.The implantation rate,the clinical pregnancy rate,the abortion rate and the ectopic pregnancy rate were compared.Results In two groups,the age,the infertility years,the body mass index(BMI),the endometrial thickness of embryo transfer day,the embryos and the good-quality embryo rate of the patients had no difference(P>0.05).But the implantation rate and the clinic pregnancy rate of MyoSure group was higher than that of curettage group with significant difference(P<0.05);the abortion rate and the ectopic pregnancy rate of MyoSure group was higher than that of curettage group without significant difference(P>0.05).Conclusion For infertile patients with endometrial polyps,the implantation rate and the clinical pregnancy rate of frozen-thawed embryo transfer afer operation on endometrial polyps with MyoSure hysteroscopic tissue resection were higher than that with traditional hysteroscopic curettage,which is worthy of clinical application.
