多囊卵巢综合征患者血清RBP4与内分泌激素水平相关性分析

Analysis on the correlation between serum RBP4 level and endocrine hormones levels in patients with polycystic ovary syndrome

目的探讨多囊卵巢综合征患者血清视黄醇结合蛋白-4(RBP4)与内分泌激素水平相关性分析。方法将2017年2月-2018年10月经临床诊断收治的90例多囊卵巢综合征(PCOS)患者纳入病例组,另选取在同一时间段行健康体检的90例正常妇女纳入对照组。采用免疫化学发光法测定其内分泌激素中的雌二醇(E2)、孕酮(P)、睾酮(T)、催乳素(PRL)、促黄体生成素(LH)、促卵泡生成激素(FSH)水平,采用γ免疫计数仪使用放免法测定硫酸脱氢表雄酮(DHEAS)以及性激素结合球蛋白(SHBG),采用酶联免疫吸附法(ELISA)测定血清RBP4。结果病例组患者血清RBP4水平和激素LH、T、PRL、DHEAS、LH/FSH水平均高于对照组妇女,E2、FSH、SHBG水平低于对照组妇女,差异均有统计学意义(P<0.05)。患者血清RBP4≥9.9 mg/L、LH/FSH≥2.24、LH≥10.5 IU/L为影响PCOS发生的危险因素(P<0.05),FSH≥6.33 IU/L、E2≥58.9 pg/ml为影响PCOS发生的保护因素(P<0.05)。对照组妇女血清RBP4与激素E2、LH、FSH、LH/FSH均无相关性(P>0.05);病例组患者血清RBP4与激素LH、LH/FSH呈正相关(P<0.05)、与激素E2、FSH呈负相关(P<0.05)。结论PCOS患者存在高视黄醇结合蛋白-4血症,与内分泌激素水平有明显相关性,且与激素LH、LH/FSH呈正相关、与激素E2、FSH呈负相关。

Objective To explore the correlation between serum retinol binding protein 4(RBP4)level and endocrine hormones levels in patients with polycystic ovary syndrome(PCOS).Methods Ninety PCOS patients diagnosed and treated in Taizhou Women’s and Children’s Hospital from February 2017 to October 2018 were selected as case group,and ninety normal women receiving healthy physical examination in the hospital during the same period were selected as control group.Immunochemiluminometric assay was used to detect the levels of estradiol(E2),progesterone(P),testosterone(T),prolactin(PRL),luteinizing hormone(LH),and follicle stimulating hormone(FSH).Gamma radioimmunoassay counter was used to detect the levels of dehydroepiandrosterone sulfate(DHEAS)and sex hormone binding globulin(SHBG).ELISA was used to detect serum RBP4.Results The levels of serum RBP4,LH,T,PRL,DHEAS,and LH/FSH in case group were statistically significantly higher than those in control group,while the levels of E2,FSH,and SHBG in case group were statistically significantly lower than those in control group(P<0.05).Serum RBP4≥9.9 mg/L,LH/FSH≥2.24,LH≥10.5 IU/L were risk factors of PCOS(P<0.05);FSH≥6.33 IU/L,E2≥58.9 pg/ml were protective factors of PCOS(P<0.05).In control group,serum RBP4 was not correlated with E2,LH,FSH,LH/FSH(P>0.05).In case group,serum RBP4 was positively correlated with LH,LH/FSH(P<0.05)and negatively correlated with E2,FSH(P<0.05).Conclusion RBP4 level is high in PCOS patients,which is significantly correlated with levels of endocrine hormones,RBP4 level is positively correlated with LH,LH/FSH and negatively correlated with E2,FSH.
