不同浓度利多卡因局部麻醉对乳腺麦默通手术患者的临床疗效观察

Clinical Observation of Different Concentration of Lidocaine Local Anesthesia for Mammotome Surgery

目的比较乳腺良性肿块患者行麦默通微创旋切术时,使用不同浓度的利多卡因局麻的临床效果。方法选择在我院接受麦默通旋切手术的180例乳腺良性肿块患者,随机分成研究组(100例)和对照组(80例),研究组采用0.2%利多卡因局麻,对照组采用0.5%利多卡因局麻。比较术中出血量、术中术后疼痛评分、手术操作时间、术后并发症、患者满意度、局麻药剂量等。结果研究组患者术中出血量低,术中、术后2 h、24 h VAS评分低,操作时间短,术后血肿形成率低,患者满意度高,单个或多个肿块的麻醉液用量较大,而利多卡因的剂量低,差异有统计学意义(P<0.05)。结论采用低浓度利多卡因麻醉可以减少出血量,减轻术后疼痛,缩短操作时间,减少并发症,加快恢复,使治疗效果更佳。

Objective To compare the effect of local anesthesia with different concentrations of lidocaine in patients with benign breast masses undergoing Mammotome minimally invasive surgery.Methods 180 patients with benign breast masses were randomly divided into the study group(100 cases)and the control group(80 cases).The study group was given local anesthesia with 0.2%lidocaine and the control group was given local anesthesia with 0.5%lidocaine.The bleeding volume,VAS score,operation time,postoperative complications,patient satisfactory rate and local anesthetic dosage were compared.Results Comparing with the control group,the patients in the study group had lower intraoperative hemorrhage,lower intraoperative VAS,lower 2-hour-and 24-hour-postoperative VAS,shorter operation time,lower hematoma formation rate and higher patient satisfaction rate.The study group had larger amount of anesthetic fluid but lower dose of lidocaine for single or multiple masses,and the differences were statistically significant(P<0.05).Conclusion Low concentration lidocaine anesthesia can reduce blood loss,reduce postoperative pain,shorten operation time,reduce complications,accelerate recovery and improve the therapeutic effect.
