高流量氧气湿化在重症脑卒中气管插管合并Ⅱ型呼吸衰竭患者中的应用 被引量:3

摘要 目的探讨高流量氧气湿化在重症脑卒中气管插管合并Ⅱ型呼吸衰竭患者中的应用效果。方法将46例重症脑卒中行气管插管合并Ⅱ型呼吸衰竭患者随机分为对照组和观察组各23例,对照组给予常规氧气吸入及气道湿化,观察组应用高流量氧气湿化进行气道吸氧及湿化处理,比较2组临床效果。结果观察组的气道湿化效果明显优于对照组(P<0.05),观察组的痰液黏稠度和并发症发生率均低于对照组(P<0.05);观察组的PaO2、PaCO2、心率和呼吸频率均优于对照组(P<0.05)。结论高流量氧气湿化可提高重症脑卒中气管插管合并Ⅱ型呼吸衰竭患者的气道湿化效果,且可降低痰液黏稠度、减少气道湿化并发症,并改善缺氧状态。 Objective Investigate the application effect of high flow oxygen humidification in patients with severe stroke with endotracheal intubation combined with type Ⅱ respiratory failure.Methods Merge the 46 cases of severe stroke line endotracheal intubation in patients with type Ⅱ respiratory failure were randomly divided into control group and observation group,23 cases in the control group given conventional oxygen inhalation and airway moist,observation group used high flow oxygen wet airway oxygen and wet processing,compare two groups of clinical effect.Results The airway humidification effect in observation group was better than that of the control group(P<0.05).The sputum viscosity and complication rates were significantly lower than that of the control group(P<0.05).PaO2,PaCO2,heartbeat and respiration rates in the observation group were better than those in the control group(P<0.05).Conclusion High flow oxygen humidification can improve the airway humidification effect of patients with severe stroke endotracheal intubation combined with type Ⅱ respiratory failure,reduce the viscosity of sputum and airway humidification complications,and improve the hypoxia state.
