连续性血液净化治疗脓毒性休克并重型甲型H1N1流感致多器官功能衰竭1例 被引量:2

摘要 1病史概述患者男性,44岁,2018年1月8日因"腹痛、腹胀、伴肛门停止排气排便4 d"入黔南州中医医院。考虑肠梗阻,肠穿孔可能,合并早期休克;既往体健。于当日急诊行剖腹探查+黏连带松解+小肠外减压+小肠部分切除吻合术。术中发现小肠扭转180°,约70 cm小肠存在广泛性砂砾样穿孔(直径约0.2~0.3 cm)坏死,坏死小肠与周边组织粘连,腹盆腔可见黄色浑浊恶臭液体约800 mL,送细菌培养。术中持续去甲肾上腺素(上海禾丰制药有限公司,国药准字H31021177)维持血压,术后生命体征不稳定,转入重症加强护理病房(intensive care unit,ICU)进一步治疗。
