初诊精神分裂症患者发生糖脂代谢紊乱的影响因素分析

Analysis of the influencing factors of glucose and lipid metabolism disorder in newly diagnosed schizophrenics

目的探讨初诊精神分裂症患者治疗过程中发生糖脂代谢紊乱的影响因素。方法回顾性分析2018年1月至2019年1月在北京市大兴区心康医院住院治疗的94例初诊精神分裂症患者的资料。按照患者是否发生糖脂代谢紊乱进行分组。利用单因素分析及Logistic多因素回归分析筛选初诊精神分裂症患者治疗过程中发生糖脂代谢紊乱的影响因素。结果 94例初诊精神分裂症患者中,27例发生糖脂代谢紊乱。单因素分析显示,BMI、Hcy水平、精神疾病家族史、使用奥氮平、两种抗精神病药物联用可能与初诊精神分裂症患者发生糖脂代谢紊乱有关(P<0.05);经Logistic多因素回归分析,上述因素均是影响初诊精神分裂症患者在治疗过程中发生糖脂代谢紊乱的独立危险因素(OR=3.888、4.075、3.504、7.775、8.297,P<0.05)。结论多种因素均可以导致初诊精神分裂症患者发生糖脂代谢紊乱,在临床中需要给予针对性的干预和防范,降低糖脂代谢紊乱的发生率。

Objective To explore the influencing factors of glucose and lipid metabolism disorder during the treatment of newly diagnosed patients with schizophrenia. Methods The data of 94 newly diagnosed schizophrenics hospitalized in Xinkang hospital in Daxing district of Beijing from January 2018 to January 2019 were analyzed retrospectively. The patients were divided into groups according to whether they had glucose and lipid metabolism disorder. Univariate factor analysis and Logistic multiple factor regression analysis were used to screen the influencing factors of glucose and lipid metabolism disorder during the treatment of newly diagnosed schizophrenia. Results Among 94 newly diagnosed schizophrenics, 27 cases had glucose and lipid metabolism disorder. The univariate analysis showed that BMI, Hcy level,family history of mental disease, olanzapine and combination of two antipsychotics might be related to glucose and lipid metabolism disorder in newly diagnosed schizophrenics(P <0.05);through Logistic multiple regression analysis, those above factors were all independent risk factors for glucose and lipid metabolism disorder during the treatment of newly diagnosed patients with schizophrenia(OR=3.888, 4.075, 3.504, 7.775, 8.297, P<0.05). Conclusion A variety of factors can lead to the occurrence of glucose and lipid metabolism disorder during the treatment of newly diagnosed patients with schizophrenia. In clinical practice, it is necessary to give targeted intervention and prevention to reduce the incidence of glucose and lipid metabolism disorder.
